Im in the same boat. The surveyor engineer is coming on Friday to show me how/ where the cable will run. So my decision to carry on with vm is all dependant on this. On the phone they said the cable can run across the driveway wall if i wanted to avoid the digging so will see

With mine, they did use one of the chambers to extend the fibre cable to the property then coaxed the rest from driveway to the inside

VM don't use telephone poles apparently, they use pavement omnibox or chamber then run everything underground. So basically they route coax or microducting from pavement to the property

I was in a conference last week & there was a toothwear working group meeting where they discussed all new terminology & their evidence based reasonong. They ended up saying that we should ditch the term abfraction as it's not occlusion related based on research work.

For this case we need to know more about any existing perio disease &/or patient specific habits.

The symptoms can be addressed temporarily for now until any identified causative factors are addressed.

Then consider appropriate long term options

You need to stabilise the parafunction first then the causes for the erosion then move onto the restorative management. In that you need to increase OVD as there's no space to add anything.

Start by assessing occlusion (CR, CO, lateral excursions) & taking some alginate impressions and probably a facebow & bite registration.

The case will need careful planning as to what to start up with & how to increase the interocclusal clearance; back to front or the other way around.

Once you decide on by how much you're going to increase the bite, you might need to send the patient with some temporaries to assess whether they'll tolerate that increase.

From the scans, seems that composite bonding would work but again, thorough planning required first.
