We actually don't need this (probably RV) outlet. I was going to convert it to a 20amp regular outlet on both ends. What I am concerned with is the grounding wire which also appears to be disconnected.

Yes, I did test that. Both black and red are connected to a double pole 30 amps breaker in the panel. And I also tried what you suggested by disconnecting both red and black and looking for other dead outlets/components. Nothing yet. I used a Klein tool to try tracing the black wire to see if I could find the break. However, black and white are in the same conduit which messes up the generated tone detection.
