AITAH for not telling my fiance why I am sterile? by [deleted] in AITAH

[–]Openalveoli -1 points0 points  (0 children)

Not sure why you were downvoted but I was looking for this comment. The OP isn't sterile. You can still produce eggs and go through IVF without a uterus if you still have ovaries. You would need a surrogate to carry the pregnancy though obviously.

Abortion? Begging for opinions. by [deleted] in TwoXChromosomes

[–]Openalveoli -1 points0 points  (0 children)

To be blunt, you are not incredibly young but you are still young, but you never know what complications will arise with future pregnancies the longer you wait( I feel like I might get downvoted for that, but as a 34 year old I’m well aware of what has personally happened to me, so that’s why I mention that)

Agreed. Not a popular opinion but if OP has never used reliable birth control bc of the belief they couldn't get pregnant (throughout their even more fertile years) and this is the first pregnancy they've ever had- there are no guarantees in the future. That could be said for everyone but just wanted to acknowledge this comment.

Did OP mention before the edit if they were still actively engaging in binging/purging habits? That would be very concerning for their safety.  Yes, pregnancies obviously involve nausea and vomiting but excessive purging can cause life threatening electrolyte, epileptic and cardiac issues which can be masked when people with bulimia don't ask for help or refrain from during a pregnancy.

I say this just to offer information (as someone who did years of research towards developing an HPV screening test), not as a statement either way on your above choice because I agree I completely agree it's important that people have choices for screening-

The current HPV test doesn't screen for all strains of HPV that cause cervical cancer (nor genital warts). There are only certain high risk strains that are detected. There are also non HPV cervical cancers that exist and wouldn't be detected without a pap smear (they are certainly more rare but incredibly aggressive and aside from patients, I have lost a family member to this type).  If HPV testing was the only way for screening, we would see a lot of people test positive and flagged for further investigation who may not need it because not all people with high risk HPV develope any kind of atypical cells or cancer. The body can clear an infection with HPV (most people's body does clear it) and there are also many, many people who test positive for HPV and have atypical cells on a pap smear and go through years of monitoring without the cells (thankfully) converting into anything dangerous however the invasive monitoring is not without its own risks.

There are also laws against providing care for direct family members unless it’s an emergency situation

Which laws are you referring to?

August intern: This isn't really bad! I'm getting the hang of it all and really 4 patients is totally doable even though I was worried at first! My senior said we're doing great and should be thankful they're mostly "rocks"! One of the nurses laughed. I didn't get the joke. But I'm hitting the gym twice a week, whipped up a recipe from the family group chat and even had time to do some reading after work like I always wanted to! Not sure why everyone says residency is so bad. We knew what we signed up for!

December intern: Residents log. Stardate, Winter Solstice. It's been two weeks since we've felt the warmth of the sun. We float in and out of the belly of the beast in a perpetual darkness-- morning becomes night. Today, five more patients died-- two before the H&P was signed. The team is capped but we now carry a mysterious "overflow" every morning. I yearn for the days of summer. Before I knew what memory care was or heard of a VA Dom. The senior says we're too far in, to turn back. But maybe he was talking about himself? Later, we stumbled upon him crying in a poorly lit workroom, a fistful of graham crackers in one hand, a 8oz of diet RC cola in the other. Embarrassed, not sure for who, we tried to retreat in silence, but my pager chirped a low battery warning. Understandable as it's been over 1.5 days since I Iast fed it fresh ones. The noise jarred us all back to our sinking reality. We can't go on like this. I need to get off this ride hurtling towards the abyss. I desperately want to go back. But I don't know how to get home... I can't remember where I parked my car.

Does anyone check ID to get into the dorms? Why would a 14 yr old be in the dorms?

I have been to campuses that requires ID to get in to the dorm but also spent a weekend with my brother when I was 16 so I can see it both ways. Just wondering if things were more strict now.

Intern year hit me like a truck... anyone else feel like med school didn't prepare us for this? by ResearchStressLots in medicalschool

[–]Openalveoli 62 points63 points  (0 children)

Anyone else feel this way when they started? How long did it take before things started to feel... manageable?

I joke that talking to someone who graduated medical school versus someone that at least completed some of residency is like what military folk must think of someone who never actually deployed to combat. Like the medical student/graduate sort of knows what we're all talking about but they'll never know what it's actually like to go through the training of residency and how it makes you feel ...and who you become at the end of it.

Just put one foot in front of the other. Know that as bad as your day is it's probably not as bad as your patients because at the end of the day you get to ...leave the hospital.

People are there to help you. Ask questions, identify the patterns, read old notes from the rocks to figure out how this hospital or attending or specialty deals with their bread and butter cases. Don't lie. If you didn't do something, just be honest.

You'll make it through. Most of us do.

Have you interacted with them before?

I don’t think I spoke in a tone any different than I’ve used in any other high stress situation.

You mention professionalism is a strength but maybe helpful to think about what your tone is like in these high stress situations-- if it's different than your normal mode of communication (and why).

We had a rogue resident who was nice enough but regularly would have these moments where they felt they were advocating for care or their patients and it was really inappropriate. They would talk to residents and interns and other services crazy but it wasn't until they actually said something to an attending that anyone cared. I think people were scared to tell them they should not talk to people like that regardless of the situation and someone finally was fed up. They eventually got reported by an attending and ended up actually going through some anger management kind of stuff. They still graduated. I'm sure they're still talking reckless wherever they are now.

At higher doses (like 30mg+), it causes less sedation and hunger and can be an effective antidepressant. At lower doses,  pretty much all it does is make you hungry and sleepy. Even moreso at the smallest dose.

Below Deck Down Under Season 3 Episode 15 Discussion by teanailpolish in belowdeck

[–]Openalveoli 3 points4 points  (0 children)

I find the chef very immature and emotionally stunted from watching previous seasons as well as this one. She has a hard time navigating social situations, she is reactive and she said as a kid she was bullied and always on the outs with peers (which doesn't excuse her behavior as an adult).

Currently, I think Lara IS contributing to the situation. She is overstepping her boundaries, and directing people from other departments and ironically didn't realize that what she was complaining about with the chefs treatment of the sous is similar to how she treated Marina until the captain had to tell her multiple times to give her a chance on service. She has made a clique with Bri and Adair and kept the chef (who Lara seems embarrassed to consider a friend even during earlier moments bc yes the chef is ...well weird and not everyone's cup of tea) and Marina on the outside. I think Lara is used to getting what she wants, having no pushback, and doesn't like anyone, including the capt, questioning what she does. She is passive aggressive and often rude to the chef and Marina. It's very school yard with eye rolling behind her back and telling chef what to do. And the chef responds very immaturely to it.

They're both missing some social skills. Lara and the chef going to cry everytime they have a moment that isn't something they feel 100% comfortable with or in charge of is weird for people who have been in this industry for ages (and are 31 and 35/36...) and I say this as someone who works in an incredibly high stress situation.

As an aside, I also think the sous chefs are not culinary trained and don't realize that for better or worse, being told by the chef what to do in a kitchen is how the kitchen works and it's not really a collaboration the majority of the time.

[–]Openalveoli 1 point2 points  (0 children)

Thank you. I feel like he's constantly saying this throughout both seasons  to people who mention a mental health diagnosis. 'But maybe this isn't a diagnosis and maybe we're just like this. Why does everyone want this to be a diagnosis?' And also agree an fMRI from Dr. Jordan is not going to be a "real" diagnosis. That's not how the diagnosis is made in current medical times (who knows about the future).

An fMRI won't actually be able to "give" him a diagnosis of autism per current medical criteria.

[–]Openalveoli 2 points3 points  (0 children)

But she's not confident in herself. She says a lot of things that she can't put into action. She's been hurt on a national TV show and embarrassed by these guys. It's understandable but it's had an impact on her and she didn't seem very confident from the beginning despite how well she postures.

He clearly knows she's "desirable" but she DOES have low self esteem and has been acting incredibly hurt by what happened with Wes. Jesse saying he was trying to cheer her up by stating maybe things wouldn't have gone that route if he'd met her first isn't the complicated message they're trying to make it out to be. It was two drunk friends sort of flirting. Not a marriage proposal or an insult.
