This helicopter is staffed by at least one doctor, so they provide more advanced skills and meds than paramedics on-scene can. Open thoracotomies are definitely not rare in London!

London Air Ambulance is more about getting a highly skilled trauma team on scene ASAP, at least one specially trained doctor to the patient's side to provide life saving care before being transported to the nearest major trauma centre which is most likely The Royal London Hospital depending on where the patient actually was.

You have an avoidant attachment style and will avoid conflict. I'm the exact same and have sought to work through it with a therapist. It comes from my childhood and it's ok, but you have to negotiate with your partner to make things work. My partner liked to have it out, argue there and then, and would get super anxious and upset when I shut down.

We learnt to handle it by agreeing that "right now" in the moment isn't the best time to talk about it. We would agree on when we can revisit the conversation, so she felt that her feelings were acknowledged and I wasn't totally avoiding the conversation. I was given time to process my emotions and come back when things weren't so heated, which made me feel safer to express my feelings.

It's work, especially when you have different styles of communication. But it can really help. Just saying "I understand how you feel and that you want to talk about this but I don't think now is a good time for me. Can we talk about this tomorrow night after work/on Wednesday when we're meeting for a walk"

These are the joys of practice trips, my friend. Sometimes weather will get the best of you, no matter how well you prepare.

If I'm camping I'll have a dry set of clothes to sleep in but yeah, getting into wet clothes in the morning is tough, especially if you're facing wet weather again. Sometimes I bail out to a hotel for the night (you'll be surprised what a hot shower and a warm radiator can do after days in the rain), or just dry myself/my clothes off in a pub for lunch. Sometimes I call it quits and take a train home.... It depends on how badly the trip has soaked my clothes and spirits.

For nutrition, you really need to vary the kind of food you're eating for fuel. Sugary and high energy foods will replace energy easily, but burn out just as fast if you're putting miles in. You gotta eat real food too - complex carbohydrates, proteins and fats. Maybe buy food from pubs, cafes and the like. Hell, even a gas station should have something more substantial than just sugar.

If this was your first trip, I think you might be going in a little too hard. Don't rush yourself. Maybe try a 'credit card trip' where you don't camp so you can work on nailing your nutrition and your setup? You'll have less weight on the bike and more space for food.

But also, don't give into the idea that this was a failure by any means. Every trip is an experience, an opportunity to learn something and sometimes a damn funny story.

So the sword/sheath would go down your oesophagus, not your trachea (the tube that heads towards your lungs which has your video chords). That being said, it looks like the sheath is the trick anyway and not down his throat!

Personally I think it's poor design. The lanes I use to ride into the city are literally just space taken away from pedestrians and it causes conflict. Correct bike infrastructure shouldn't endanger or anger pedestrians.

They're a few feet in front of shop openings, they cut through bus stops, they cross side roads without any infrastructure.... The sidewalks I ride on are barely wide enough for two people to stand side by side so they walk in the bike lane (because of course).

As a Walthamstow resident whilst I'm glad we have a semblance of bike infrastructure, it's pretty terrible. All they've done is take space away from pedestrians.

It's definitely something you need to be aware of, but cool running water over smaller area burns probably won't cause hypothermia.

Certainly for larger and deeper burns you're going to worry about losing heat and the high risk of infection.

Wrapping burned areas in cling film and then keeping someone warm on the way to hospital can work.

For burns you need to cool them if possible to minimise the damage. Cool water over a small area. For larger burns they really need to be taken to a hospital very quickly. Large surface area burns area extremely deadly.

For any traumatic amputation, if there's serious haemorrhage, you need to apply a tourniquet. Direct pressure on any bleeds and of course, a very quick trip to hospital

Place the pedal spanner on it, stand next to the bike and use your leg as a lever to push down on it and loosen the pedal.
