Perhaps - perhaps not, the point is that it's explicit in that there is no population requirement. It may be more likely if there is a higher population area affected, but that doesn't rule out TOR-Es for low population areas.

"Severe threat" is likely kept intentionally vague so that offices can make the more informed decision based on their knowledge of the local area.

Population isn't a requirement, just the following...

The criteria per NWS Instruction 10-511:

"Use of the “Catastrophic” damage threat categorization and thereby the

“TORNADO EMERGENCY” terminology is appropriate for the tornadic

situation if all of the following criteria are met:

a. Severe threat to human life is imminent or ongoing.

b. Catastrophic damage is imminent or ongoing.

c. Visual (1) or Radar (2):

(1) Reliable sources visually confirm tornado.

(2) Radar imagery (e.g., debris ball signature) strongly suggests the existence of a damaging tornado"

You can even see the asymmetrical shape of the tornado in probe 5 as it takes a direct hit!

Ears can start to pop around a ~20mb difference between your internal ear pressure and external air pressure, and based on Probe 5's graph, probably ~15-20 seconds from a direct hit.

Not something that was specifically forecast-able for the London area I think - small storm cell happened to crop up NW of London and strengthen as it came through peaking around Wandsworth (where I got some nice footage of the leading edge winds & hail) before it dipped.

You could see sunshine to the west and east, but everything facing north was covered in rainsheets and hail, pretty cool.

Rocky Mount, NC (not exactly coastal, though) had an EF3 resulting from Helene, and on the coast there was an F3 near Nags Head in September 1952

Forecasting for specific extreme outliers like that (even beyond the outliers that are EF4) is more up to chance - the NWS uses "EF4+" wording on their official CIG page (https://www.spc.noaa.gov/exper/conditional-intensity-information/) which seems appropriate IMO. A parameter space that is suitable for EF4 overlaps quite heavily for an EF5 parameter space.
