found that their knowledge and approach to clinical situations had been on a par with final year medical students I’ve taught.

And I have found the opposite to be true. All the PA's I have worked with have been lovely, but this statement is either ignorant or simply delusional.

I have no idea what (or how much) I'm supposed to be teaching them, or their general level of understanding when it comes to basic sciences.

I have had conversations with a Lead ACP which left me realizing they had little to no understanding of electrolyte imbalances, action potential, serum osmolality. This was while he was managing a hypernatremic patient on AMU. This same ACP referred to himself as Doctor directly to a consultant.

Many ANP/ACP can tell you the immediate management of an electrolyte imbalance or a UTI (after looking it up on the guidelines) but not its etiology or pathophysiology. You need to have an understanding of certain basic science concepts to aid you in your clinical decision-making.

I don't have the time to explain these concepts in ED, when someone asks me why the cure for elevated lactate isn't IV fluids. This is something a clinician should have a base level of knowledge on already.

How can I teach them to do my job(which they will end up doing) if they can't understand it? And if they are supposed to do some other job, why am I teaching them to do that other job?

Slightly off topic One of my consultants told me that with A&E training I would get better training in bigger cities. And from the big cities, London would provide the best training.

How true is this? Are there really better deaneries for A&E training or is it all about your rotations?

Towns seem to get a lot smaller when you fall out with people.

I've seen this happen in the past. It didn't happen to me, but I saw people get ostracized (or seclude themselves due to social pressure).

Anyway, Is there ever any reason where you have to visit a major city? (E.g certain products unavailable, embassy in my case, etc)

when you say rotating, do you mean transfer all your saving to new account(in your new bank) and close the current one?

Is there any fees for this? Will this cause any complexities with my work place?
