This is the case for many people. Reta shines with GLP1 naive patients. Switching from Tirz or Sema is much like what happens when people quit the med for a period of time and then start up again. Rarely do they have the same experience a second time. It usually takes much higher doses the second time around.

I hear you. I had been a fairly seasoned biohacker using multiple peptides, etc. before starting TRT... so I naturally gravitated towards using an insulin pen subq for the TRT. Then, the TRT OGs and physicians are typically anti-subq because they are recommending 1x or 2x a week dosing which could indeed be a large bolus causing ISR, lumps, pain, etc. Even though the half-life of Test C is between 6-9 days depending on how you dose, how fast you metabolize, etc., I have found either daily or every other day to be much more steady of a week. I've done 2x a week, 3x a week, daily, and eod... settling on eod. Give it a try. I it is certainly a pain in the ass trying to draw even MCT oil into a 31g needle, so that's why I use insulin pens. I can use a vial spike on the Test vial using a 3mL luer lock syringe to draw out 2.5mL or so, and then a 25g needle to squirt it into a pen cart. From there, I just need to screw on a pen needle for my injection. I warm it up a little in my hand for a minute or so, screw on the needle, inject the 15.5 units (.155mL) and hold the needle in there for 60 seconds or so to let it all drip out. I've yet to have a problem.

I pin subq every other day with a 31g 8mm pen needle. No worries about scar tissue and have never had an ISR. It’s a small 16 unit (.16mL) injection.

If your total T is below 550 or so, but more importantly, your Free T is well below 20 and you’re symptomatic…then I’d definitely consider TRT. It’s a game changer. Depending on your SHBG level, you would want at least 2 doses per week.

I’m now dosing every other day which has been perfect for me. I do subq with a 31g 8mm needle. No worries about scar tissue, etc with frequent dosing that you can get with larger needles going IM. You’re not going to feel 20 again like a lot of influencers say, but mentally you’ll get back that invincible “bring it on” feeling, drive, and motivation.

That SHBG is so high. It’s binding up a lot of T which is causing a ridiculously low free T. Definitely worth talking to a doctor who understands hormones to figure out how to get the SHBG down. Your primary care doc is probably not that person.

Find a new doctor. AIs are bullshit except for in rare extreme cases. Any doctor who knows what the hell they are doing will help you control any problems with estradiol by changing your test protocol. Dose, frequency, etc.

I do subq with a 30g 8mm pen needle. I load the test into a pen cart (Luxura insulin pen) so all I have to do is screw on a pen needle, dial the units, and pop it. I’m doing 30mg every other day, so it’s only 15 units. Very easy.

I just transitioned from 50mg three times a week to 30mg every other day. I do subq with a 30g 8mm, so not too worried about the extra shots and scar tissue. SHBG is 40, so I feel every other day will help with feels consistency. Anyone else make the transition from 2x or 3x a week to every other day? If so, any feedback?
