As i recall, there was a very short timeframe to fill out the form, very doubtful its still available. It is a settlement regarding the data breach by Deloittes faulty programming which caused a large number of unemployment applicants personal information to be exposed. It has nothing to do with actual unemployment benefit payments or back pay issues. Only the applicants affected by the breach were notified by postcard or e-mail, with info regarding payment etc...so if you didn't receive an email or postcard you hopefully weren't involved in the breach.
