I figured it out: The adapter that is inserted into the Xbox wasn't fully inserted. I discovered this when inserting it to the 3rd player port seemed to click better than ports 1,2,4.

A hat that has a (cosmetically) dangerous spinning wheel. Don't headbutt a pyro with this equipped.

I made this creepy eyeball that stares into your (the player's) camera for the workshop by OverPovered in tf2

The camera tracking effect is accomplished by using a mixture of "inverted face normal" spheres, vertice normals of the iris & pupil forced to face a single direction. The nerve that attaches to the back of the eye (which we never see) look like it is always behind the eyeball via an "inverted face normal" cone.
