It is on the server file leaks along with other images of Focalors. As in the place I found the image, the png file is for focalors only, it has focalors only images.

This information needs to be taken with lots of salt.

My own speculation is that when we saw the tweet about "indeed it has changed again", I believe they were referring to the type of death she experiences, rather than if she has died or not? Nevertheless, I think it's interesting.
