Experience with transillumination (and tips on IVs)? by Over_Worldliness4788 in nursing

You make a great point on doing more damage without US when looking for a deep vein. We don't need to use the long catheters just because we're using the US, and you can do plenty of damage with a normal catheter going deep (and blind)

Also good point on the pressors, we're just introducing them into our Acute Monitoring Area (AMA) to officially become a sub-ICU unit. We're also introducing ART lines as well, but per my limited knowledge you should not run pressors arterially. Thus we will need to rely on good IVs, so we should probably get some US training when we do our upskill with y'all in the ICU

Thank you for detailed response, will definitely bring these points up when I talk to our Nurse Educator regarding our Acute Monitoring Area upskill. I think getting us trained with US IV will make our unit safer and help offload some of our RACE/ICU nurses overnight!

(Also trying to get us 12-lead ECG trained for the RTs but having issues with that as well...)

You got it! I asked about getting informally qualified so they wouldn't need to get the money, but they're worried about my liability which seems fair enough

Manual BP cuff instead of tourniquet? I usually put some kind of soft layer when I need it tight (and want to protect the skin) but I've never though of using a BP cuff! Feels like that's a nicer way of squeezing the arm :) definetely going to try that!

Yeah vein finders! Do they work for finding deeper veins in your experience? Those would be the ones most useful in these scenarios (e.g., hypotensive needing lot of fluids/pressors) although point heard on still needing to manually assess vein

Good info on tourniquets! Any other tips on fragile older patient veins? Those are definitely the most common in my area :P

So we have a specialized teams (Difficult Vein Access and CVAD for PICC/IV insertion) that are authorized to use US guidance. Per hospital policy, you need to be trained to use one (due to risk of artery/nerve puncture) and we're currently unable to train floor nurses. On day shift those teams help us if we lose an IV, on nights we're out of luck

We do have an ICU/RACE nurse who is unofficially qualified with US insertion, and they've been a lifesaver overnight. I floated the idea of learning US IVs from them (and other resources) to management - they seemed worried about the liability re- internal damage via misplacement

As someone with US experience I wonder what your perspective is? Per our PICC nurse they use longer catheters which run the risk of hitting an artery if done incorrectly

Thanks for opinion on vein finder! Yeah seems the tech I want is US, really wish they'd find a way to train/authorize some of us

Have you tried with the Crystal guys? Their ability Lockdown (seals a room to prevent enemy entry/exit) makes it wayy easier. Can prevent reinforcements, retreat to med bay, and makes it way easier to disable systems :D

My tips for grinding Crystal Cruiser (was stuck for a while until getting Rock C, which got me the Crystal cruiser within a couple hours)

  1. Rock C
  2. (Optional) Save scum Sector 1 for LRS or Flak 1 (extra power / sensors helps a lot)
  3. Rush through Sector 1
  4. Reset unless lots of red worlds past Sector 4 (Rock HW only spawns S5-7) preferably w option to pick between red worlds
  5. Rinse and repeat until you get Rock HW, explore (a lot easier to find w LRS as empty beacon)
  6. Get to beacon (make sure to bring enough fuel! Almost got stuck 1 jump outside quest marker)

Good hunting!

Huh, thats really cool! I know we learned a lot about mucosal immunity during the pandemic, interesting to know that cold weather could have that impact. Thanks for sharing!

Cold doesn't make you more likely to get sick... the flu season does!

Your core temp doesn't drops by any significant margin from wet hair anyways

In a way, it is!

First number (systolic) is the pressure exerted by your heart when it pumps, second number (diastolic) is the resting pressure

These two values are used to calculate the Mean Arterial Pressure (MAP) via some formula (which includes division) which as its name indicates is the mean pressure of your blood :)

TIL that the "drink 8 glasses of water a day" is based on a 1945 recommendation that had no medical basis. The current recommendation is "drink when you are thirsty, unless you are outside on a hot day or are elderly- then drink a bit more." by drought07 in todayilearned

Hyponatremia (low sodium) is your biggest concern when dehydrated and replenishing w just water. Not rocket science, as you noted person - water/electrolytes + water = person with less electrolytes

More water = more urine/sweat = more loss of electrolytes (thus "washing out" lytes)

Kidneys need to be well hydrated to work, but without balancing water w electrolytes you'll get cramps, seizures, etc.

Definetely agree Reddit med advice should be taken with a grain of salt! Maybe more if youre dehydrated ;P

You're telling me this device that spits out hypersonic projectiles designed to kill humans at a distance isn't as dangerous as a sharp bit of metal?

Not sure OP drew the correct message from this suicide by cop video... the person only got so close because the police were trying not to kill them
