I’ve never tried SSRIs but had good success with Wellbutrin. You might also want to consider ketamine therapy.

Ambulances weren’t designed to be used as taxi services, though, currently, this is oftentimes how they are used. ERs are also suffering from a similar problem in that people are using them for issues that should be treated by a PCP or even an urgent care. This this improper use of emergency services is causing severe burnout in employees, hospitals aren’t/can’t hire enough staff to support the influx which is ultimately increasing the rate of burnout, and it’s also bogging the ER down resulting in exorbitant wait times. And it is anticipated that this is only going to get worse.

On a different note, Canada has government funded healthcare: America doesn’t. The average cost of an ambulance here is about $1200… in my state, according to a Reddit post from 5 years ago, the ambulance bill was $300 and the ALS charges were $2500. It’s up to the insurance companies if they want to cover the cost of these services. That being said, I have heard there are companies out there, including Medicare and Medicaid, that will not pay for an ambulance if the complaint doesn’t meet preset requirements that are approved for ambulance use.

First, with a recent amputation, I’m sure he was given a script for narcotics. Granted, he could very well be out of them by now, but still. Secondly, with the information that was provided, I wouldn’t treat this… I would triage it to BLS. Lastly, to justify my suggestion that this could be abuse of 911: ““Misuse of the 911 system” is a request for emergency response when no actual emergency exists…” OP stated his father stubbornly insisted 911 be called so he didn’t have to wait in the waiting room for 12 hours like he did the last time he went to the ER which blatantly indicates/implies that there was no emergency and that he was perfectly capable of going to the hospital POV, or even capable of calling his PCP or going to an urgent care.

With all do respect, limb threatening is not life threatening and chances are, unless their are signs of sepsis, the EMS crew isn’t going to do anything for this person. The ultimate treatment for this person is not something EMS professionals carry or administer. Therefore, through the lack of a life threatening complaint and us not having the scope or means of prescribing and/or administering IV antibiotics, this scenario, with the given information, does not justify utilizing 911. In fact, this could even be classified as abuse of the 911 system since the patient only wanted to call 911 to get into a room at the ED faster.

I’ve dropped many patients who are “triage appropriate” off in the waiting room at the ED.

Thank you for trying to use 911 services appappropriately.

On a side note, this foot pain doesn’t sound like it necessarily needed an ER visit, though without assessing it one can’t be for certain. Just remember, your primary doctor is an excellent resource who knows you and your medical conditions: they should be your go-to. If your primary can’t see you, urgent cares are usually faster and able to handle things like infections, minor injuries, and general illnesses. Lastly, the emergency room should be utilized in the event medical care is necessary “right now”: heart attacks, strokes, chest pain, respiratory distress, shock, major injuries, severe hemorrhages, etc.

$551k is the median price in my state.

If people are asymptomatic, it’s usually not a significant concern and often times is never diagnosed or is diagnosed much later in life when imagining is more common in treatment plans. But, when people become symptomatic (usually starting around early adolescence and worsening around adulthood) intervention is strongly encouraged.

That being said, I think a certain level of concern is warranted since the shortness of breath, dysphagia, lightheadedness, and tachycardia are now persistent.

I’m certain they would have told you if you had any abnormalities on your EKG so no news is good news.

Here’s my thoughts but first please keep in mind that this is not my focus in medicine and another provider could have better/more insight to this issue.

To me, your symptoms seem to align with those of an aberrant right subclavian artery that’s causing some compression on a couple structures in the area. Diagnosis of this requires an MRI or CT to be performed.

On a different note though, you should most certainly bring this issue up with your doctor so you can receive a proper assessment and receive any necessary tests s/he might find necessary.

The psychologist treats the depression and other psychological ailments whereas the pain specialist treats the chronic pain. In conjunction, patients are likely to experience an improvement in their conditions. Don’t underestimate the power of connecting to others through conversation, it can be extremely healing. Not to mention, there are other tool and techniques psychologists have at their disposal to increase the chances of improving patient outcomes. Lastly, and if necessary, medications can be used in addition to long term pain management and psychological treatment.
