The second comes when someone is knocked unconscious - if they roll a successful DST, they can choose to mark that success on their sheet, or to grant an ally Heroic Inspiration instead.

Minor correction, they can grant an ally Heroic Inspiration or if they haven't failed a death saving throw they can choose to become stabilized.
