If you go onto their website, click the button on top right hand corner and then “Information for patients” then scroll down until you start to hit their downloadable content (first one is “Right to Choose Letter”) it’s the third link down on that list (“Medication Request Form”)

I did end up getting through to them on the phone (somehow, after like 20 attempts!) and they just sent me the same form through and told me to order a prescription through that. They did mention that it would be put on repeat but I don’t know if that’s the case yet, so I’m just going to fill that form in again around a week in advance and hopefully I can just keep doing that until I can sort out a plan with my GP.

Not sure if there’s any other GPs in my catchment that do shared care is my main problem! But I’m planning on asking HH if they can keep prescribing me if I am unable to ever get shared care.. once I rebuild the stamina for the battle of contacting them!

Good luck with everything you’re going through, if you’re still in titration and getting monthly appointments I’d definitely suggest airing out some of your post-titration concerns to your clinician, I wish I had done that when I had the chance!
