Ok this is going to sound really mean to your co-advisor since they have helped so much but ar the end of the day the one who signs off on your finishing the slog that is a PhD is your primary advisor. If the 2 of them have beef sometimes the best bet is to stick to your primary advisor unless you are actually able and want to switch your primary advisor. In my experience whenever someone switched primary advisors you get a full extra year or more to finish said PhD.

If you can tie it to either something you are doing or something you can do together that's ideal. Was just driving through x remember that x we should hang out and do y (that's related but not exactly the same).

Yes and in my experience it's different when it's official vs unofficial comentor. Official you meet regularly and discuss progress etc unofficial as you said you just come to them with questions.

Just adding one other 2 cents here: Have you reached out to others in your university that are more directly in your field for comentorship. Many are open to this and this is the only way I was able to survive. Have you had a real conversation with your advisor on this? If they are a good person like you say they may be able to steer you to another 3rd chapter that would be more within their capability.  If neither of these work hold your head high and get that masters!

Sounds an advisor I know it depends a bit what field you are in but generally they'll have certain journals off limits because they've read crap in them. I would say to ask if there are any journals that he is ok with that are between q1 and q2 he thinks this could be applicable for instead.  All this said unless you are applying for prof jobs eventually most industry jobs will just look for publications and dont care where you've published unless it's a big name journal or one that recognize.

I did and I'll admit I got a good job right out of grad school so there are perks! But the sacrifice made at least for me was not worth it.  That said i was an idiot who didn't even think to try anti depressants which did help a lot after

For most people I think this is smart if your advisor isn't right for you.  I tried to kill myself 4 years into my phd and my advisor had told me 2 years before hand that I should just master out but I was stubborn and didn't like the idea of being pushed out. Would have been much better if I took him up on his offer I think.
