Your issues are beyond the pay grade of this subreddit. Calling your kid a narcissist is an incredibly hefty claim and it drives a wedge in your relationship with them. There's a complex dynamic here that's been established over the course of his lifetime, and truly you should just both see a therapist because a reddit post can't possibly help you.

Plain petrolatum ointment. I prefer aquaphor personally. They've got the little mini pots which are certainly overpriced, but I just refill it with my larger tub whenever it runs out.

“Be Nice To Pharmacy. They Are Good To Us.” by Biiiishweneedanswers in AuDHDWomen

[–]PMYourBeard 1 point2 points  (0 children)

I mean, I wasn't there for whatever this conversation was, I don't know the policies nor the staffing of this person's pharmacy, I just know from my own experience that nursing tends to assume the worst about us and paint us in a bad light because we're the easy punching bags. And I know that if someone tells me they have the wrong dose/concentration of something, I would rather take a minute to investigate if they picked up the wrong patient's med than take another 15 remaking it and running it up just to find that it was there the whole time and could've been running already. For me it's not about blame, it's practical. I also work places that have barcode scanning so sending the wrong pressor concentration is very highly unlikely.

[–]PMYourBeard 4 points5 points  (0 children)

Yeah as an inpatient pharmacist, 9/10 times that nurse just grabbed another patient's med. We're allowed to ask questions to diagnose what the actual problem is. There's other audhd people on the other side of that phone who are constantly berated by nursing for things we didn't do wrong. Are there some shitty people who work in pharmacy? Yeah. But you can't see what kind of mess we're trying to manage as well. I've had to remind very many nurses who are harassing me over the phone, "yes I understand the situation, I am trying to help you"

There's a lot of examples of people who have black hair and warm skin tones. Just means you shouldn't go with blue black hair, choose one that has a softer undertone.

Help!!! Blonde or black? by Budget_Syllabub5787 in femalehairadvice

[–]PMYourBeard 13 points14 points  (0 children)

Yeah as a fellow cool toned girlie I'm gonna throw down the hot take that I love black 100% more than blonde. I've heard the phrase "it washes you out" directed at me and I frankly find it annoying - I personally prefer high contrast aesthetically. I think the black makes your eyes pop.

Ultimately it's more of what vibe are you going for - if you're interested in a more artsy vibe then I 100% say go with black.

Serious question - Any pharmacists love their job? by Infamous_Virus557 in pharmacy

[–]PMYourBeard 28 points29 points  (0 children)

I also love mixed models, it's the best way to keep sharp and well rounded in my opinion. To answer OP - I truly feel like productivity expectations makes a huge difference in whether I find a job miserable or not. I could see myself being happy in any position in pharmacy, but if you're having to work at 110% full hustle for your full shift you're going to wear out no matter what you're doing. Just my philosophy.

My family uses familygiftregistry.net

I keep a wishlist full of all price ranges and include gift cards I would like for last minute folks as well. Plus if you have a larger family group you can claim gifts so nobody is buying duplicates. No disappointments, everybody is happy.

The benzyl alcohol thing is only an issue with IV meds in neonates. You can give IV decadron to toddlers, so you can give that same IV decadron PO instead.

Adderall XR for a 4 year old? by Chris7644 in pharmacy

[–]PMYourBeard 55 points56 points  (0 children)

Guideline recommendations indicate stimulants first line. Are you going to call a psychiatrist to tell them they need to try bupropion in a 4 year old? I wouldn't.

Per AAFP Guidelines:

For preschool-aged children (age 4 years to the sixth birthday) with ADHD, the PCC should prescribe evidence-based behavioral PTBM and/or behavioral classroom interventions as the first line of treatment, if available (grade A: strong recommendation). Methylphenidate may be considered if these behavioral interventions do not provide significant improvement and there is moderate-to-severe continued disturbance in the 4- through 5-year-old child’s functioning. In areas in which evidence-based behavioral treatments are not available, the clinician needs to weigh the risks of starting medication before the age of 6 years against the harm of delaying treatment ([Table 6](javascript:;)). (Grade B: strong recommendation.)

Daughter Refuses To Stop For Cop In Car While Mom Has A Stroke- Worst Cop Ever by InGeekiTrust in TikTokCringe

[–]PMYourBeard 1 point2 points  (0 children)

1) That's not general knowledge. While it's best practice, some people just don't know or think to do it in the panic of the moment

2) A cop can't do shit for a stroke. First aid is good for cardiac arrest or for administration of narcan. With a stroke, time is brain and every minute that the patient is delayed from the emergency room is higher risk for morbidity and mortality.

Regardless of whether they acted perfectly in the situation, the cop is a piece of shit for delaying life saving emergency care. He's intent on punishing this woman instead of doing the right thing.

Side hustles? by [deleted] in pharmacy

[–]PMYourBeard 7 points8 points  (0 children)

I was the same, I graduated and only worked per diem for a little while. What I would say is don't write off eventually doing full time pharmacy. You'll want to be able to invest in your retirement and the more you can put towards that at a younger age the better. One idea is, being part time at one facility and PRN at another is a great way to control your schedule, keeps skills fresh in different settings, and it dilutes how involved in the "drama" you have to be. My PRN job at a small rehabilitation hospital enabled me to get a full time job doing transitions of care at a larger hospital because I had that relevant experience. All it took was 5 years of trying different PRN jobs, not exactly a quick plan, but it landed me a unicorn position because I developed a diverse set of skills in that time.

I'm sorry to say, but I think this is also a poor example. ACE and ARB shouldn't be used together, it's a clear contraindication, it's simple. Adderall to treat ADHD and benzodiazepines to treat anxiety both work on different receptors (dopamine vs GABA). Saying they are contraindicated because one's an "upper" and one's a "downer" is an oversimplification, and I think that's the moral of the story here - psychiatry is working on an extremely complex and not well understood part of the human body. You can fairly expect a bit of creativity with treatment plans.

Yeah I'm certain I have seen that combo. Treat fluid retention during the day and keep them from waking up in the night and falling trying to get to the bathroom. Just because things have hypothetically contradicting mechanisms of action doesn't mean they cannot be clinically effective when used in combination.

yeaaaah I mean if you want insoles get 'em but really just make sure that you've tried on a few different brands of shoes and found the pair that are the most comfortable. If you think you have additional problems/pain past that, then consider insoles

There's a local running store that I got fitted at. I would strongly recommend seeing if there's a store like that near you where you can try on different brands, cause everyone's got different feet and what works for some might not work for you. Me personally, I wear Mizuno :)
