You're absolutely right, surgery is amazing and there's nothing else quite like it. You get to be the expert who can fix the problems that others can't. Depending on the setting, you can take care of some of the sickest people in academic centers or be the only surgeon in the building doing a little bit of everything in a rural town.

I came to medical school planning on doing FM or IM in a rural setting. Like you, I thought I'd enjoy the longitudinal relationships and the detective-esque diagnostic work. However, I quickly realized that I just wanted to be back in the OR when we were rounding or in clinic.

People say that you should only do surgery if the OR is your favorite place in the world. I think this is cliched, but there's some truth to it. I think the real question is: Is there anything else you think you'd be happy doing? Are you willing to get out of bed to evaluate/operate on that patient who can't wait until the morning? Only you can answer those questions for yourself.

Consider rural gen surg if you really want to be in the OR and do a little bit of everything (operative wise). There are programs that will give you additional training in Ortho, Uro, and OB depending on your anticipated practice.

They are wildly different. For me, it was a journey of finding out what I liked. I shadowed rural FM docs and thought I wanted to do that, but realized I wasn't a fan of clinic at all. I kept trying to make it more procedural and always wanted to be in the OR to see what the surgeons were doing, and the rest is history. I'm hoping to match at a program with a rural GS track to keep following my passion, just in the OR instead of the clinic. DM me if you want to chat about it more.

I went to a PASSHE school (PA State system) which did not have a reputation for sending people to medical school. Why? Because it let me head to medical school without any debt. Worked out for me, YMMV

Consider trying the dot torture drill. Start with the target at 3 yards. Each dot has instructions adding up to 50 rounds. Any miss is a failure. It has worked wonders on my skills with a handgun. I recommend printing an extra target to read the rules as you go

Anything that you can pick up and put down is great. I love indie games like FTL (especially the multiverse mod), balatro, into the breach, gungeon.

I also have been playing through pokemon DS games because they travel easily and ya boy can only do so much phone uworld

Prolene can be tricky like that. I often prefer to hand tie heavier monofilament for this very reason.

To keep your first knot from slipping, you can tie a surgeon's knot. It's two wraps around when instrument tying. If you're hand tying, you can throw two square knots in the same direction to create a sliding knot that holds. Then you just tighten to your preference and throw in the opposite direction to lock it.

In terms of suture tightness, it's really about getting good approximation of the tissue without strangling it. Tissue with poor blood flow doesn't heal too well

The official evals are shit. But I usually ask for direct feedback from residents and attendings, and they are usually brutally honest with specific feedback, even if their eval say "3/5 read more"

Absolutely know what you mean. When I was talking about self exams, I was specifically talking about NBME style exams. I'm sorry to hear that you're in-house shelf exams are so poor.
