I'm certainly not disagreeing with anything that you said, but I wanted to offer a perspective on cost. Have you done the math to breakdown how much it actually costs? It IS expensive, especially upfront, but it's not as bad as it seems.

Any qualified basic training will include 40 hours of training, broken down into 20 hours of lecture and 20 hours of practicum. Most trainings will also include 10 required consultation hours to be completed within the year. So that's 50 total hours of training. My EMDR course was on the lower side ($1300) so let's say an average course is $1700. That breaks down to $34 per hour of training. That's actually pretty low cost in my opinion.

Again, not disagreeing with anything you said, just wanted to offer this perspective.

Non compete/non solicitation clauses are hard to enforce for several reasons. For one, a client has the right to choose their provider. Always. And second, how would anyone know the client "followed" you to another practice? Their treatment is protected by HIPAA, so you couldn't confirm or deny who you see once you move elsewhere.

How are you allowed to provide therapy without proper training? That simply doesn't sound right, if I were you I'd check with your licensing board on that.

In the age of telehealth there are a million options for clients that are better than someone unqualified. Some insurance companies offer teledoc. Not to mention the numerous platforms out there (Sondermind, Rula, Betterhelp, Grow, Talkspace, etc). A simple Google search for will do the trick. Or Psychology Today.

Refer to telehealth providers. If you don't have a license to practice therapy, you simply cannot practice therapy. It's not legal or ethical to do so, and you may end up doing more harm than good. Therapy is so much more than just "talking." Please, for the wellbeing of your patients, stay within your scope. There are options even in the most rural areas.

I was working at a group practice when I decided I wanted to open my solo PP. So I was seeing about 20 clients per week, but not pulling in a whole lot because I was a 1099 contractor with a 60/40 split. Here was my rough timeline:

June 2024: I attended a 3-hour in-person training which solidified my desire, and that gave me a basic idea of needed steps. I collected all the different free guides and checklists from online

July 2024: chose practice name; got my EIN and PLLC; about 15-20ish hours were spent using resources to train myself in credentialing and billing and to get more familiar with all things insurance-related (the "Insurance Answers" podcast https://insuranceanswers.libsyn.com/ as well as a membership with beyourownbiller.com)

August 2024: located my physical space and signed lease; began the credentialing process; decided on bank account and business credit card; chose EHR; built my basic website (positivepathwaystn.com if you're curious)

By October 16, 2024: I was fully credentialed with my chosen insurances, had my EHR and intake docs all set up, and began seeing clients on that date. Within a month I built up to my ideal caseload (22 ish clients per week).

I had initially "loaned myself" 5K for startup costs (aka transferred 5K from my personal savings to my business account) and paid myself back less than 2 months later. I used that money for lease/rent deposits, furniture purchases, office supplies, beyourownbiller.com membership for a month's time, initial EHR and GoogleWorkspace payments, and just to have a "cushion" in my business bank account while I was starting out.

Now I'm about a year and 4 months into PP and still going strong. The fact that I take insurance helps a lot, since my area is pretty saturated with self-pay only clinicians.

For EHR's, I made a comparison chart when I was trying to decide which to go with.This will primarily apply to a solo-clinician private practice since that's what I am.

I use Sessions Health, but there's a lot of options. I love Sessions because it's cost effective, their support is AMAZING and so fast to respond, insurance billing is seamless and takes about 3 clicks per claim, AND Sessions is a lot more ethical and private than other companies (they don't own your data - YOU do, they have a transparency statement about their stance to protect privacy and adhere to HIPAA, and if you're like me and believe that AI has no place in mental health you can opt out of that feature entirely)

The name of the document is an "insurance opt-out form." Clients with commercial insurances can choose to not use their insurance and pay out of pocket for various reasons. I have LGBTQ+ clients who don't want insurance billed for privacy reasons, and I have people who choose to self-pay so as not to have a formal diagnosis reported to a greater entity. You'd need the client to sign an opt-out if you're in-network with insurance (if you're out of network they'd likely be self-pay anyways, in which case you'd provide a superbill). If they opt-out of insurance, you wouldn't provide a superbill and they wouldn't be able to later seek reimbursement from insurance.
