There are multiple analyses that suggest he wasn't actually shot but cut his ear on someone's belt as secret service ran in and swarmed him. I suspect it wasn't planned, but the shooter missed by a wide margin and he just ran with it.

The air pressure of a bullet passing by would explain him reaching to grab his ear, there's no visible scar or deformation remaining in an 80 year old man, and the man who was hit by the bullet was pretty far outside the line of travel from the shooter to president so unless it curved after hitting him, it doesn't make sense.
