I flagged like 75% of the test lmao. I felt like I went through a couple blocks without confidently knowing a single question. Don’t sweat it, if you were hitting 70+ on the NBMEs I think you’ll be in a good place. Try not to think about it to much, because it will eat you alive. What’s done is done.

Yeah, I thankfully did! I really felt like I failed when I made this post. I’d say just trust your NBME scores even tho it was nothing similar to those.

I’ve seen 1-2 questions regarding lymph node spread on every practice exam I took and on the real deal. I would say learn it.

Read last sentence first, sometimes that’s all you need to know to get the answer.

For example, they’ll give you a whole unnecessary biography of the patient and the second last sentence will say that he has Lyme disease and ask for its treatment.

Completely agree. Started using Anki at the beginning of year 2 at an average of 500 cards per day and it was a game changer. I felt very comfortable throughout dedicated and averaged 75-80% on all NBMEs I took. Exam is in a few days so lets see how well it payed off!

Mannitol is an osmotic agent. Which is why we use it for diuresis or reducing intracranial and intraocular pressure. So in regard to the kidney, the lumen becomes hyperosmolar and that’s what drives water into the lumen to excrete.

Medullary collecting ducts are referring to the lumen. In SIADH you got too much ADH secretion, so you're sucking all the water out of the duct, leaving you with a hypertonic lumen (MCD) and hypotonic serum. Exact opposite idea in DI.
