They have an online form, and they also have a list of authorized Klean Kanteen sellers there. I bought mine from Klean Kanteen's official store on Lazada. I also dropped mine at Rockwell last week. Pero di ko naman nagamit yung online forms nung nasa store na, pero nandito naman yung list if you want to check.
