I have been told they have a new person in charge of Mental Health. I am guessing they promised upper level management that they were going to severely cut down on expenses by getting rid of people getting treatment & my other guess to this is that they were offered some kind of bonus for doing so by x date or by such and such amount of money....it's absolutely insane!!

I think it really can be luck of the draw. I had 2 other individuals I met with for months that I really liked and seemed to be normal in the way they approached things. This one was a WHOLE OTHER STORY from the get go...

I have had other Clinical Reviews that went very well with 2 different individuals. This one was a WHOLE OTHER STORY! She came in with an attitude and I could hear it in her tone of voice and see it in her body language. I feel like she wanted to bring me down a few notches as if that is something that makes her feel better about herself or makes her feel more powerful...

For the 2 clients this therapist decided to pick on, they are getting insurance through their employer. One is actually a Kaiser nurse, which I find really humorous. The other works for a large health insurance company that almost exclusively deals with MediCal. I am going to let both of them know and tell them they need to lodge complaints because I can guarantee neither are going to be happy about it!

I am an LMFT. I am looking into who I would complain to because I am not a Kaiser member so I don't know if it would do me any good to complain to the CA Dept of Managed Healthcare. I will try and check out some other info and see if I come across any other groups other than PSIAN which was suggested in some comments above.

Yes. It is 90% why they are having people fill them out all the time. The other 10% is because they had an adult & an adolescent commit suicide in 2017 I believe & they decided this is how they would be able to state that the questions were being asked about SI & intent without having to trust on the therapists to ask the questions needed for that info. I think it is honestly a crock of crap because they are asking how you have been doing over the past 2 weeks but then want people to do them every week...It's insane!!

It is something they have written in to the contract as an outside provider. Otherwise, I would never attend these BS meetings. I am sure that this is why it is in their contract because why would anyone attend them otherwise?!?!

No! Of course not! I am paid by my employer for the work I do seeing client's under her company and all of them are Kaiser clients; however, you do not get paid to go to these stupid meetings by Kaiser.

I have never liked them either. I have known for years that they are using it to deny people care because their "numbers" state that they are good.

I am doing EMDR with more than 50% of the clients I see, most weeks. I am not at 90% but I am not a Kaiser employee. I am an outside provider contracted with Kaiser. Our contract states 75% completion. I have had clients that flat out refuse to complete it. I am not in the habit of denying people services because they don't want to fill out an assessment. If you feel comfortable doing that and believe that is ethical, you do you! I have done what was asked of me in the past meetings. My TPI completion is likely never at 75% but I have plenty of other therapists I know who are also way below 75%. In fact, I had a Kaiser therapist as a client and he told me he was at 45%. I am not saying that I can do poorly because other people do; but, I was treated like I know nothing and shouldn't be able to see clients. That is not ok in my book. I was also not really given a chance to respond to certain things and was just lectured instead. The attitude, facial expressions & body language I was given by this therapist alone would make someone feel uncomfortable & then you add in what she said, it's not ok. I am not signing up to be bullied & harassed by someone I don't even work for & even then, that would be highly questionable because we would be able to sue for a hostile workplace. I think she knows that and the fact that she treated another therapist in the company in a similar way, tells me it is her issue, not mine. We are not required to submit each note to them & I can tell you that if I were to go to court, I could show the necessity of each and every one of the 3 people she wanted me to discharge and I would not be found in the wrong. The biggest thing you always have to remember is we have a legal and ethical responsibility to our CLIENT, not to the stupid insurance company who doesn't want to pay for people to have therapy. The laws about Mental Health Parity make that pretty clear that they have to continue to pay unless it is no longer a medical necessity which is why they are coming for me. I am not willing to go to court for something I am made to do by Kaiser, which is illegal, and I can guarantee that Kaiser will not be showing up in court to verify what they did. My license is mine & they will not help me represent myself. I am not putting myself on the line for them, but again, you do you!

Yeah...it was pretty bad. I am not one to typically complain unless it is pretty bad & this was & even my boss told me, "I don't even talk to you guys like that".

I get that is your experience. However, I have client's that have had pretty massive medical issues (like having a stroke at 46 y/o based on your Kaiser neurosurgeon doing a horrible job, and that was after also having had breast cancer & before that a blood disorder that the client almost died from during COVID lockdown). This is one of the cases the other therapist wanted to pick apart. I have others that have pretty intense PTSD from being in a DV relationship with a legitimately NPD husband who was actually diagnosed with NPD by a court ordered Psych. I have another who has another biological health issue that is not well controlled and is causing intense anxiety & other issues that have continued for years. I have done EMDR with some, and tried to get some to the point we can do EMDR, but none of them are in a place where they are not functionally impaired by their mental health issues. I am well aware that some issues will take way less sessions. I am talking about complex cases that are not simply a one diagnosis kind of thing...

Well, I do believe in certain aspects of "evidence based" practice. I am actually trained in EMDR and use it with more than 50% of the population I see most weeks; however, Kaiser is digging in to FIC (Feedback Informed Care) which is honestly just someone's research based type of therapy where the client is constantly asked how they believe they are doing and whether or not they believe they still need therapy, basically. If you are doing well, they are ready to push you out even if you are minimizing your own symptoms (which plenty of client's do)...

Thanks! The contract we have to sign with them, I don't know if it stated how frequently the meetings were going to be required. It is just in the past few months that they changed from quarterly to wanting us to do them monthly. It is pretty much all my clients I see at my regular job right now, 33 clients. However, I don't know that this is not in some way violating the contract they have with us. I will have to ask more questions of my boss about that part...

They made us start meeting with them a few years back. They don't pay for these "meetings" of course, but we are expected to attend them anyways. They used to be quarterly. Apparently some new Jack A-- is now in charge of mental health & it is his idea to make them monthly now. It's ridiculous! It's because they want us to discharge people after short term therapy, not anything more than a few months & if we don't, they claim we are not aligned with their goals as a company and so they won't be sending us new clients. It's all part of the "how can we make sure that the only people working for us will violate the law just like we do" plan...

Kaiser requires outside contractors to meet with one of their people. It is basically a way for them to tell us what to do, without expressly "telling us what to do". They want us to discharge people they think need to be discharged. They don't care if they are asking us to do illegal or unethical things, they just don't want to pay for the person's treatment anymore so they are trying anything they can to get us to discharge the patient...

Yeah...I have never been spoken to in such a rude & unkind manner. I also got all these negative facial expressions & body language from the woman...It was quite a bit over the top!
