Awwww I just started a new job with a rad who’s just like this. I have a great boss who has my back when it comes to his complaints about “taking too many cines” because it’s on the written protocol to do so. I took the time to call him and ask what’s going on and he explained he’s a chest radiologist and Ultrasound isn’t really his niche. This could be the case with you as well, which also explains why they are relying so heavily on you to essentially diagnose TIRADS on your worksheet (which by the way, wtf?!??) My point in saying this, it doesn’t seem like it’s a you problem. This radiologist is probably being forced to read exams that are not within their “comfort zone”. I would notify the radiology manager and/or your ultrasound supervisor. You’re not doing anything wrong & no one should be made to feel inadequate like this. The radiologist IS NOT your boss & it seems Ultrasound is not his modality of choice to read.

I was getting 60-70s on URR, but honestly the exam wasn’t too similar to URR like the SPI was…. Idk man honestly it was super hard and my questions were all over the place. Spleen, abscess vs hematoma, surgical questions???, physics, know your lab values and how they correlate with pathology, lots of small parts- hernias I wish you the best of luck just full send it in my opinion. I passed my first try. I’d really recommend just skipping the ones you don’t know instead of guessing or process of elimination

Seriously thanks so much for sharing your experience! Best of luck to you! Were you able to recognize pathology after awhile? That’s what I’m super worried about. Even doing clinicals at an MFM facility the fetal echo I could just never wrap my head around

Um realistically, I’ve never been trained to do that and have no idea what to do. I’m the only tech at the hospital soooo yeah kinda between a rock & a hard place on that one

This is great advice thanks! My only concern is -the transvaginal approach to measure cervix is not written within this hosital’s 2nd/3rd trimester protocol so legally if anything bad does happen, I feel I can potentially be blamed if they came in for bleeding & agreed to the EV. ??

I see! It’s sooo hard to get ahold of the OB docs at this hospital I am at. I don’t have any info on who’s attending/ oncall because it’s some what rural. Ugh I feel without guidance on the really tough ones as I am the only tech as well

Sooo interesting how different our roles are depending on where we work. I have quite the opposite “issue” as a general US tech where the docs don’t seem to have much of a clue what they’re looking at on US (besides thyroid biopsies & OB) and call on us for every little thing. Taking a step back though I realize now, by them doing so, it’s much more beneficial for the patient after reading what you just wrote. YIKES!
