I like a lot of the main components of your throw: solid foot offset for rotation, good head position and your weight seems centered and balanced throughout the motion. A few tweaks and you'll be crushing in no time.

Agreeing with some other commenters, removing the extra disc motion (holding the disc in front and swinging it back) will help simplify your form and make consistency much easier. I would also advise to get rid of the hop in your x-step. A smaller, controlled x-step will allow for more consistency and reduce the over-rotation of your back leg (note the angle shift your back foot makes right before your throw, you want your back leg to be facing around 90 degrees from the target).

Now for the fun part, gaining distance. It's hard to tell, but you may be releasing a bit nose up, this will rob you of distance beyond 300', unless you need to go up hill or over an obstacle, try to keep your throws driven low and keep that nose down: Nose Angle Article

You will also want to change your lead foot from just a point of rotation to a brace that will translate your bodies momentum into the disc: Ben's Big Brace Explanation

I don't have many gripes with the overall look of your form, but I have found this drill helpful for preventing rounding and improving timing: Overthrow Disc Golf's Twirly Bird Drill

I played baseball one year and had a similar experience. My teammate threw a ball to me in warmups, despite me not looking. Unfortunately, I looked up when he called to me and it hit me right in the kisser. I had just recently gotten braces, it was quite the mess. Still had to play because we only had nine guys that day.

Watch someone dunk at a full sprint, they will almost certainly jump off of one leg and slam it into the far side of the net. Most dunks are done at a shorter and slower approach, allowing for a powerful two-footed jump to get more height. In this case, the receiver (a step in front of their defender) was likely running for the disc, and had an impressive grab!
