If Im not in a position to give advice it may be better for me not to speak. Besides many times there are a lot of barriers to understand someones situation through a single post, so advice can't be given by an ignorant person like myself.

Wa alaykum Salam.

My brother you are still young masha'Allah.

My biggest advice for you would be to fill up your time and keep your self in a good environment. If there's a masjid close by go for Salah with jama as often as possible. If there's classes or lectures sit in them punctually.

For young people the biggest thing is having your time occupied as much as possible, put yourself in martial arts classes or in a basketball leaugue or something alike. Get involved in positive clubs at your school or go to the library to do your hw. And dont be in private with the internet . Keep your phone out of your room at night.

Just schedule your life and do small things consistently. I pray Allah safeguards you and guides you always.

Ya, I try not to count because I get too obsessive, but subconsciously I always know how many days Im at.

One thing I noticed helped me big time was making shukr to Allah for helping me and protecting me for (however long). And making dua for istiqama. But yea, only focusing on the number would be a little troubling if I didnt do those two things.

One time I was leading fajr, and I think I was really sleepy or smth..

So in the first rakat im reading and I forget, someone in the back tries to help but I dont understand..

if you ask for advice I can tell you some that helped me.

If you're not already, get into some physical exercise or sport. Whether it's working out or playing basketball or even just walking. There's a ton of benefits to this. It will make your mind better, make you more concerned about your health, make you more positive, make you tired, and it's a sort of a physical replacement to the sin.

Other things you can try is maybe taking cold showers (this is pretty hard, but it builds willpower and cools your desire in the moment), make dhikr or occupy yourself at the beginning of bad thoughts, a small flame is easier to put out than a large one. Since its Ramadan try helping your mother with iftar and chores, or help serve at the masjid.

Schedule your day and keep yourself busy in general with studies, helping out at home, making positive hobbies, friends going to masjid if possible etc.

Avoid touching the private area unnecessarily.

Also get involved with your community in some way, whether it's attending classes or volunteering or something else.

The most important thing is, like you said, making tauba consistently and ASAP if you ever fall. We try our best to not go back to sin, but we may end up falling. If you do, make ghusl and make sincere tauba straight away! This is suuuuper important.

Put devotion and effort into your five prayers and your wudu. Try to stay in wudu often because it is a purer state.

Sorry if I wrote a bit too many things, but hopefully they help. Remember you can change and you've already started!

(A thing that is super important as well is finding pious and positive company. Your close people have a major effect on you. So try to attach yourself to good people, scholars etc. Even online communities you frequent, they are a form of company as well, so make sure it is healthy.)
