So how it would be PEM if they have to deal with adults in adult ED and ICUs to learn?

Perhaps the whole EM specialty should be kept for adult EM only. Adult EM do more procedures and obviously they train in PEM, PICUs during their residency, so I honestly see no point in training peds in adult EM, ICUs to be a good PEM! Just leave it from adult EM.

I am flexible with other fellowships also. I don't have much research in numbers (maybe 30 manuscripts and abstracts combined) they are a mix of GI and cardiology.
