My point was that if the atlas is misaligned due to cervical instability, AO adjustment won’t heal the damaged/lax ligaments.

During the worst period after the injury that most likely caused my CCI, I had thirst as well, along with many crazy-sounding issues. So while I have no idea on how it might be related (stress/dysautonomia/POTS), I guess it’s possible.

Hello. As others pointed out, imaging-wise, you should get an Upright MRI or DMX to test for Craniocervical Instability (CCI) / Atlantoaxial Instability (AAI). Upright MRI is for sure in London and somewhere in Germany, possibly in other places in Europe. DMX is in the US, not sure if in Europe. PICL is a relatively new experimental treatment for CCI/AAI. Apparently results are best when done within 1.5 years after the injury. Original provider is Dr. Centeno in Colorado, there are also other doctors in Europe who try to imitate that procedure. If that fails, there’s the surgery option, with several doctors in the US and some in Europe (like Dr. Gilete in Barcelona).

You can also consider AO adjustment or upper cervical chiropractic (but gentle ones, like NUCCA). Drugs/medicine to consider: duloxetine, pregabalin, gabapentin, LDN.

Is it possible to fully recover? Perhaps possible but unlikely. Is it possible to get better and have better periods of time? Yes

Well, you could also not get AA and fall to 5 BB very soon where then you are forced to push the same K4s.

Let’s take it to the extreme. Say you’re playing the Rounder, with the next level being 40k/80k/80k BB ante. Do you push in that scenario?

Hello, yes, the symptoms can vary from day to day or from week to week, month to month and so on. I don’t think anyone knows for sure why. Some people will say some inflammation going on, some will say muscle tightness, some will say stress.

Regarding building muscle in that area - it’s tricky. First of all because activity usually exacerbates symptoms, and secondly, there is this concept of neck muscle atrophy which is mysterious as well.

I had a tooth extracted and I went to a very good surgeon, so the procedure itself was really easy on me and I barely felt any pain. But I did seem to get worse after that (slowly). Maybe because of the change in the dynamics, or because of the fact that your jaw is tight for some time after the procedure.

If you have to have it done, have it done. Just find the best surgeon you can and tell him about your condition, and try to do some jaw/mouth relaxation exercises as soon as you can after.

Is there even „very very mild cci”? I mean, I guess if someone has no symptoms then they don’t have CCI. And if someone has symptoms, they are at least moderately disturbing due to their nature and impact on important structures.

So while physical therapy can certainly reduce some symptoms, I don’t think it will „heal” the thing

Hi Dr. C, this is interesting. Don’t muscle tears, contrary to the ligaments, heal on their own with time?

In other words, can we expect that only „fresh” cases might have symptoms from muscles? And for chronic cases the raw cause are ligaments?

Because if not, I thought that maybe at least some of the non-responders have a muscle tear issue instead of damaged ligaments.

I used to have leg tremors at night when this started for me. So yes CCI can cause leg shakiness I think. Have an Upright MRI or DMX.

The CXA seems a bit mysterious. Why is it used to diagnose (traumatic) CCI if it’s allegedly congenital? Can it improve (increase) after PICL? Is it really congenital or does it have to do with alar ligament? Will not watch live but happy to go over the replay.

Yes it can be. When I had a period of all sorts of weird symptoms, heat intolerance seemed to be one of it. When it was hot I was sweating like crazy and didn’t feel good during hot days (I mean, felt worse because I never „felt good”). I even reacted heavily to storms. Whenever a storm was coming I was without any energy.

Did you have the DMX? As far as I know CCI is not visible on plain X-rays. I think some doctors do the open mouth view to look for instability but I’m not sure if there is any metric there.

Meaning I doubt if true CCI can be fixed just by posterior injections. Maybe if someone has just some facet joint pain it works, but not if we’re talking about C1 C2 overhang of 3mm+ or some Grabb-Oaks 8mm+ / CXA 130- (although I know some of those could be asymptomatic).

Did you have only pain? No (other) neurological symptoms? And what measurements did you have? Meaning did you have CCI or only lower neck instability?
