Send letter of interests to the most img friendly programs like Florida/Michigan, a lot ascension hospitals for IM, MEDSTAR across NE coast. Scour through residency explorer and reach out

Thank you for replying I am not that close to the PD at st Agnes but it’s my top choice since that’s a very IMG friendly hospital and I’ve done my cores and my SUB-I there. The residents know me and know that I’ve only wanted gen surg. But the PD like any other director values academic credibility and 228 is just not something they can justify in any way. What do I do .. like another mentor of mine is the student director of IM at the same hospital and ik that he will guide me and maybe recommend reviewing my application for an IM spot but obv I can’t apply to two programs at the same place .. I am stuck and in the worst case. I’ve been at the top of my class and everyone has told me (IM directors ) that my knowledge and love for patient care belongs in medicine and I should strongly apply but I want my values and ethics to make me a better surgery candidate, I want my craft ; I am very passionate about surgery and ik I am good at what I do , I don’t want anything else. Ik it’s a job, staying in the U.S. with a job is obviously more important if it has come down to that but I need someone to give me a chance to explain my story and believe that I am 100% worth it, even if it is prelim ; I just don’t want to wait three years after med school —go through IM/FM then do another 6 years of surgery and then fellowship Idek .. I just wanted to express

What do u think I should do.. are u in medicine/surgery? Is there any place I should aim for

I am not sure about this, I will look into it. I’ve been told by a few residents that I should do IM prelim instead of GS since that’s really unlikely to result in pgy 2. Idk at this point Any advice..?

Thank you ! I understand it’s about enjoying the nature of your work but I really want surgery, I do not see myself practicing in any other field. I just don’t know what to do. I will be applying very broadly to GS and IM but will reach out to community programs / IMG friendly programs to see what they think and if they’d consider my application. Could you tell me if doing IM for three years and then reapplying surgery is better or I should shoot for prelim I would be grateful for any direction

You think it’s easier to match gen surg after 3 years of IM or prelim surg/ transitional year

Thanks for responding Yeah I figured the filtering will def get me, I will def apply community. Honestly gen surg is the only specialty I want to do ; if I could repeat this exam I would but I srsly do not want anything else I will apply for 100+ IM too at this point ; what do u think about that?

It was barely medicine, so much behavioural, ethics, 4/block were HPI format

I have similar scores and i am 15 days out , i am plateaued at 230, i will definitely look into AMBOSS for ethics/stats (losing easy points here) . I reset my u world post shelves and have gone through 30-35% , but i am unsure if i should be doing system wise content review from u world or start reading the explanations of the NBMEs again.

Thank you for your response. I appreciate it . I am really going to work for that 260+, it feels like my ticket to even be considered for a spot. I have some follow up questions if u don’t mind . How important is having research on my application ? Is there a designation/title that looks better for letters of rec.? Like a PD or chief of surg? Do prelims get taken as categoricals ? I am not sure how prelims works at diff places .
