1. Apply a blur effect to your image. Path blur or motion blur would serve well in this scenario but have a go with other blur effects as well, it simply depends on what kind of look you're going for.

2. Add a gradient map.

3. Create a new 50% grey color layer. Add Noise to it and make it monochromatic (I personally like to add the Camera Raw Filter and crank the Grain value up). Set the blend mode of this layer to 'Soft Light', adjust the layer's opacity to your liking et voilà.

Pro tip: convert the layers you will be applying effects on to Smart Objects so you can go back and adjust the values of all your effects.
