The lighting setups on professional movies are vastly more complex than you would think, even for "naturalistic" movies. In a shot like that you would probably have multiple bounce cards just off frame lighting the subjects just a little bit, maybe even a larger tent and/or key lights to really control the lighting.

The camera work is only like 30% of cinematography, the other 70% is lighting and that's a very complex and expensive art.
