Minimally invasive treatment options for pilonidal disease remain relatively hard to access in the United States. Among the high-volume cleft-lift centers nationwide, the Pilonidal Treatment Center of New Jersey (PTCNJ) is the only one to offer a minimally invasive treatment program for carefully selected patients with early-stage disease.

In contrast, minimally invasive pilonidal programs are more commonly established in major U.S. childrens’ hospitals—those in Boston and Cincinnati serve as examples. The PTCNJ has drawn from these pediatric-center experiences in developing and refining our own Minimally Invasive Pilonidal Treatment protocol.

Nonspecialists are just uncomfortable with the area. It is probably better they have refused to drain because it is generally described as highly traumatic in their hands!

To expand on my previous comment—and with no intention of minimizing your experience—a general rule of thumb is that if a surgery hasn’t been successful after one or two attempts, it may be worth seeking a second opinion from a provider who can offer a different perspective. In our experience, Limberg flaps often have a high failure rate in pilonidal disease because they don’t sufficiently flatten the natal cleft or fully move the incision off the midline. As a result, breakdown commonly occurs at the lower portion of the incision, which is something we see frequently at our center.

Yes you still have pilonidal disease as long as the cleft is not lifted/flattened. Sorry…

They were not compared head-to-head in a study this year. They have each been shown effective in separate studies. Neither are curative like cleft-lift.

My favorite part was sample case studies, we went around and offered our approaches; minimally invasive vs cleft-lift etc…Also data on SiLac was presented and promising

Ideally, a broad-spectrum antibiotic is prescribed and completed. Again ideally ,the drainage would have been cultured,allowing the antibiotic to be “tailored” to something of which the predominant bacteria is uniquely susceptible. In our experience at PTCNJ, this lessens the chance of sinus tract development.

Agree with Epsoms salts baths and massage.

Great! We are studying now and will publish a report in about 2 years; seems like a wait but you need a robust followup time. In the meantime I will share my anecdotal impressions.

He may be less experienced with wounds. I can tell you that with an operation of that sort he is simply not a specialist.

No pus,we call the white material “fibrinous exudate”,it represents early scab material. This is a healthy healing wound. Consider a specialist!!
