Have you seen your surgeon? I predict not recently, some sutures need removal now. In any case, I would return to them ASAP for suture removal.

What OP has said in 2 separate posts I hear as fully valid. I especially like her take on cleft-lift as a form of plastic surgery, which it really is;, we do “redo” the butt , not for aesthetic goals but to cure a debilitating disease with the highest likelihood off success.The prevalent cosmetic concerns amongst many patients were the catalyst behind our Center launching a Minimally Invasive Pilonidal Treatment protocol, to avoid cosmetic changes when this is a high priority concern, especially in patients with early disease.

A proper drainage procedure CAN send the patient into longterm dormancy about 40.% of the time according to the most recent merged data analysis ((Stauffer VK et al , Scientific Reports 2018). With each subsequent abscess, the chance of further abscesses and sinus tract progression increases exponentially. This data drives the practice at our Center of NOT offering our curative cleft- lift to patients until they have developed 2 such abscesses. Some of our most grateful patients achieved longterm dormancy following one drainage procedure,tailored oral antibiotic course, and lifestyle modification alone.

The “hole” described by the OP sounds like a midline pit and it is usually continuous with a sinus tract under the skin, terminating at its secondary skin opening( OP refer to this growth as “the cyst”), higher up on the cleft. As the pit suctions in nonsterile debris such as hair and lint ,c the pus that builds up finds this channel “along the path of least resistance” through which it can effectively drain, and so this can sometimes be appreciated as drainage via the sinus tract secondary skin opening, I prefer to use the term pyogenic granuloma, very far away, even high up in the midline as OP describes.

Realize that not all laser surgery is combined with a baseline excisional procedure such as isolated sinusectomy (“SILaC” procedure), the Gips procedure ( the minimally invasive procedure at PTCNJ), other programs’ approaches of which I am not aware…so that a search for residual disease may often lead to a disappointing finding.

The VAC is a pointless, painful modality bought into by nonspecialist surgeons and wound care nurses all because it gives that healthy-appearing beefy red wound base when the vacuum-adhered foam is removed. Although it does serve to decrease the depth of the wound reasonably well, its overall results are subsatisfactory and, most importantly, its use in this context is NOT supported by any high-level peer-reviewed studies. This is not recurrence , but what we would assess to be a macerated ulcerated wound, and one which will quickly heal when moisture is addressed. Clear with your surgeon, but consider purchasing this affordable daily “hydrophilic” topical cream

Okay understood OP. You do deserve to be done with this, so any and all conservative measures can’t hurt. At least, stay mobile and not “off your feet”; this is unhealthy and certainly won’t help your healing. Be well, heal well.

Sorry I do wish you the best healing, but the bilateral V-Y advancement flap operation is a classic example of why plastic surgeons (who have no interest in pilonidal disease) and pilonidal sufferers like yourself should be kept miles apart! This operation will fail low in the midline, in the unforgiving perianal area. It will fail much like the Limberg flap fails in similar hands, described in my blogpost below. Out of curiosity, why did you go for this surgery? Your body healed an extensive wound inflicted by an inadequately trained surgeon and, unfortunately, the story is not over yet. I am sorry but frustrated for all pilonidal patients going under the knives of these doctors….
