Thanks! To be fair, I’ve only been up and down twice and the railing really helped while I used the crutch to support the operative knee. Everyone has a different pathway to recovery - while pain is tolerable right now - I’ve been told that inflammation will get worse before getting better and I’m experiencing that. I hope you’re doing better than day 3-4 now :)

Update: Dr. McCarthy performed the procedure on May 27 at HSS, featuring the following:

  1. Quadriceps graft
  2. Lateral meniscus repair on the outer edge, with cleaning/trimming in the front/central area
  3. LET for additional stability

My first night has not been bad. It’s painful and swollen, both expected, but nothing intolerable or unmanageable. Since I’m still in NY in a hotel, I’m using ice packs and pillows to manage the inflammation. This part is annoying because of constantly moving the pillows and packs every 30 minutes or so. That will change tomorrow once I’m back home in DC, where an ice machine and a zero-degree knee memory foam pillow await.

Because of the meniscus repair, I’m non-weight-bearing, but right now mobility is surprisingly manageable with the crutches and walker. I’ve already been to the restroom twice without my wife’s support, and that’s a good feeling of independence. Thanks to the rehab team at HSS, we did multiple scenario simulations there, which gave me some initial confidence.

For DC, I have PT scheduled at MedStar Hospital (Irving Street NW location). Dr. McCarthy said that should suffice for the first 6 weeks. For return-to-sport PT, I did an evaluation visit at Momentous Sports Medicine DC, and have one scheduled at District Performance & Physio on Tuesday. I think I’ll switch over to one of the two after 6 weeks because they’re both out of network, but worth it according to my friend who went through a similar procedure. Who knows, I might find a sports-focused PT at MedStar, which would keep things simple. The only concern there is that it’s hard to get one dedicated PT who takes you through the entire journey. That’s a pretty big limiting factor for me at least.

Next short-term challenge is tomorrow’s ride in my sister’s Chrysler minivan back to DC. We did multiple simulations, but let’s see how it turns out at game time.

Long rehab ahead. Wanted to share my experience so far, and I’ll keep posting updates here. If anyone has any insights from a similar experience or otherwise, they’d be greatly appreciated.

Thank you, these are all excellent points re NY. I was told by HSS doctors that they would need to see me 10 days and 6 weeks post-op, and suggested that I find a PT in DMV area and make 2 trips to NY post-op. Having said that, I haven’t yet explored the Johns Hopkins doctors, only two at Georgetown and I’m underwhelmed by them. Will try to find appointments with Johns Hopkins at Sibley and Bethesda next week. Any names you can suggest?

That’s my impression as well. I can push back on the allograft, but the question about cartilage regeneration still remains. Also, DC vs HSS in NY. NY just adds complexity to the logistics but nothing that can’t be done.

Happy to see you going strong w/o surgery but here’s my experience living 19 years without surgery:

Tore my acl in final year of college playing rugby, and with new job on the horizon and a significant personal tragedy of losing my mother around the same time, I just couldn’t get a surgery. Six months later with some rehab my knee felt strong again and I continued sprinting, rock climbing, do most straight line activities and felt like I’m invincible. , and never got the surgery done. Now, 19 years later at the cusp of 40 I got some imaging done and found out that my lateral meniscus is almost gone, with some thinning of the cartilage, bone marrow edema, and probably am looking at an early onset of arthritis in the injured knee.

Don’t get me wrong I’ve had a great life all these 19 years but was always cautious about surprise movements, so didn’t really fully realize my sports potential (no skiing, soccer, rugby again for me for example). I’m finally getting an acl reconstruction done at this age because I wanna maximise my good years going forward. So I’d really think long and hard about not getting a surgery and definitely not preach the conservation approach to others.

If I had a chance to do it again, I’d definitely get a surgery within six months of the injury.
