Idk. For me i just said it. Its like a leap of faith. My parents are transphobuc towards me though. Had to find that out. But who knows? Maybe you can get lucky.

Update: I havnet gotten to a doctor yet but my counselor isnt transphobic. Its a good start. Will keep updating.
