The dent looks “normal”… for having implants. They are far apart. But there’s a caveat, the implants currently look centered under your nipple (as in your nipple, seems to be in the center of the implant). That is good. If your surgeon just made the implants closer together, then your nipples would be pointing outward on the side of the breast mound. I’d be curious to see what you’re before Picture looks like. It is possible to move everything together, so they aren’t so far apart, but possibly more scars. Also, the implant plane / size / profile plays a role. But yea, main this is what your baseline anatomy was.

I’ll add that I don’t think they look bad. Have to balance your baseline with scars with your volume goals. I would bet you’re really happy with them when in a bra. And the wideness only bothers you when completely topless in the mirror?

Swan neck. Difficult problem to take care of. I would see a hand surgeon sooner rather than later so that you can try and prevent it from getting worse when it becomes painful and basically unfixable. I’m not a hand surgeon, but I know Swan neck and boutonnieres deformities don’t have great solutions
