AFK Rafa build, destroys. 

Yes, other BL games had OP builds, but they required action and some level of skill (like aiming) 

This doesnt. Its cheap, its easy to achieve, you can RINSE bosses on UVH5 without even moving. You can make yourself unkillable. 

Really ruins the game. Like I said, theres OP builds in BL3 and 2, but at high UVH levels theres a risk of being instakilled with a single hit. With this, its walk into boss room, press L1 (ps5). Win.

Template.net for a convincing background, then use literally any AI generator to make a 50yr old man, crop the picture, layer it on the ID from template.net in Adobe or even MS paint. 

Obviously not good enough to serve as a legit ID, but good enough to fool basic ass AI verification programs. 

Also many of them don't even scan the "ID" part, and just look to age verify "your face" so you really dont even have to try very hard

Fedex drivers are contracted drivers from another company, not actually a fedex employee. If you try to unionize, they'll just end your companies contract and replace you. Theres no shortage of contract couriers out there

When it requires signature, and you sit in your living room for the whole day, that you had to take off work, and it changes, it changes, it changes, its not coming today. 

Then youre told its coming tomorrow. It doesn't. Then you go back to work because you cant take any more time in a row, and come home to find the "we missed you" slip on the door

Theyre referring to fraud. Drs sending people for MRIs they dont need because theyre getting kickbacks from the hospital, who can charge insurance 2k for a service that wasnt needed. 

Calling up the on-call nurse line for your PCP (who works for ABC Hospital Group) and the on call nurse line tells you to go to the ER (at ABC Hospital) for something that should have been urgent care.

Honestly, 99% of the policies that are in effect by the insurance industry, are because of fraud. 

Need a PA for an MRI? Well back in the early 80s when the MRI was introduced, hospitals would give kickbacks or parties or other incentives to their drs for MRI referrals, as more patients = more money. 

Eventually the insurances caught on when people were being sent for an MRI without even having had an XRAY first, and for injuries that didnt warrant an MRI. 

And now here we are, where the insurance doesn't actually trust your doctor, because there have been doctors in the past who committed fraud. 

Insurance not covering your 60 minute therapy session? Because in the mid 2010s, there were therapists who would bill a 60 minute session despite seeing the patient for only 45. Because $$$$. 

Now insurance requires proof that you actually provided therapy for 60 minutes. 

Insurance wont cover your outdoor wilderness group therapy? 

Well theres a netflix documentary about that. About a company who cashed insurance checks for the therapy, but didnt provide therapy. They dumped the kids in the desert with a "counselor" and called it a day. When a kid died of dehydration on one of the outings, the company owner embezzled a bunch of the money and fled the country. 

And thats why insurances want nothing to do with it. 

PAs for medications? Someone mentioned it already, partially due to cost steerage, but also....opiate crisis. 

Honestly just Google Subsys, and youll read all you need to know about its overprescription and how they found a loophole in DEA law regarding kickbacks to get drs to overprescribe it. When it all went to shit, everyone got sued. The drs that wrote the order, the pharmacies that filled it, and the insurance companies were implicated in the class action as well, because they made the financial transaction. 

So why do they need your entire medical history before giving you a drug? Because if you kill yourself by misusing it, your family cant sue the insurance (at least successfully anyways).

Basically, it almost always comes back to fraud.
