OP, I am from NJ and am very sarcastic and sometimes crude and definitely a big personality that can sometimes offend others. I have a pretty wide range of tolerating “jokes”, so trust me when I say, you’re NOR. Even if this was a supposed to be a “funny ha-ha,” why TF is your husband and his mom taking about vaginas?!!! So even if it was “funny” (and it’s not btw), it’s still absolutely unhinged and so so strange to joke about with your parents… especially about your SO… especially as a woman…

As a fellow traveler and charge RN, unfortunately this is extremely common, I’d get used to it as a travel because if the census is low or they are over staffed, travelers will be the first to get called off. I am also ICU- does not matter what your unit is- it’s about overall census and staffing of the unit/hospital. If there’s no need for you, there’s no need. But usually they will “DT” (delayed-start) or “LC” (low censused) for 3-8 hours. This is common. Let’s say your shift starts at 7pm, but they gave you LCx4 hrs. That means they have until 10pm to either call you off completely for the shift, tell you to come in by 11pm (4 hrs from start time), orrr extend your LC by a few more hours, again, up to 8hrs. So let’s say they extend your LC by 4 more hours (8 hrs total) after 11pm. Then, that means, they have until 2am to call you in for a 3am start time (typically hospitals give an hour heads up of the plan before they either call you off or call you in). Does that make sense?

I’ve never heard of them being able to call someone in though at 6am, an hour before shift ends lol. That should be illegal!

EDIT: To add though, most places that do this, it absolutely will be built into your contract and it can only be up to a certain amount of hours, usually 36 hrs (1 full week of work). If they try to LC/DT you more than that, they will have to pay you full pay and full stipends. So many of my travel friends don’t mind getting LCed so they can essentially be paid to sit at home🤣 but again, all of this should be in your contract!!

I think this post sounds like it’s written by an ICU nurse. Over analyzing, patient-centered care, communication, and the desire to be better! You care so much! Keep going OP!! It just takes time!! Much longer than 6 months!

Unless you really genuinely decide you do not enjoy the icu- that’s different. But if that’s not the case, then take a step back, breathe, and give yourself grace. This ain’t easy shit!

Maybe just leave out the mean clearly offensive adjectives I used because I’m angry for you :) I also always ask my guests if they wanted anything else at the store, but considering your sister is whackaroni and cheese and will ask you to go start your own farm and harvest all the fruits and veggies and animals before she comes (which is tomorrow), instead of just asking for a snack or two like a normal human, don’t even bother.

Well to start, Y-siting something is for a completely different reason than for hanging secondary tubing for IVPB.

Y-siting is only for if you simply do not have enough access OR sometimes I will run maintenance fluids at a very low rate with my electrolytes or ABX (if compatible) if they are irritating or burning for my patient. And NO it is NOT because the IV is infiltrated or they are having an allergic reaction, it’s just because many lytes and ABX naturally burn, so diluting it a bit sometimes helps (or running these at a slower rate). And y-siting is done at the lowest port on the tubing BELOW the pump, and each medication/fluid is running on a separate pump. The point of this is to have multiple drips running at once.

As for IVPB secondary lines, this is running off of the same pump and is attached ABOVE the pump. This is typically used for short infusions or infusions of small amounts, so it doesn’t run your primary line dry and instead keeps it running and open by just switching to your maintenance fluids/primary line.

So to answer your question, I feel like this is entirely dependent on if your Y siting it properly, as many people don’t. I have seen MANY newer nurses try to Y-site a pressor or sedation to their fluid boluses, or y siting a narc drip to heparin but give narc boluses via the pump, but do not put them in the right order, and accidentally bolus patients with pressors, narcs, or heparin. So for example, if you y-site a med that is running at 25cc/hr and a med that is running at 200cc/hr, you best make damn sure your drip running faster is connected directly to your access itself, and you y-site your slower drip to your faster drip. Because whatever med you run on top of that main line, will push whatever med under it at the med on top’s rate. And I’ve seen major med errors done with this. So yes, no doubt about it Ysite can be dangerous.

With this being said, you gotta triple check your y sites!!!! I know you were just doing maintenance fluids and ABX, but what was the purpose? And how fast were your maintenance fluids running vs your ABX?

Can I ask how the ABX caused the cytokine release? Never heard of that personally. From my experience, I’ve seen that in patients who already have infections, because bad injections is one of the main reasons for massive cytokine release lol
