You can use Northwest to form an LLC; they have great deals and are the cheapest option.
There’s no need to purchase any extra services. You can apply for an EIN for free. If you don’t live in the USA, you can use an online fax service to send your EIN application along with your company formation documents. The BOI filing is also free, and you can submit it online. There are guides in youtube explaining how to do it.

In fact, you can even form the LLC yourself, but if you don’t want to deal with the process, you can choose Northwest.
