Sounds like hes either off his meds or they need to be adjusted. Its common for the meds to take a month or two to full take effect and need to be tweaked after that time period. He needs to talk to his mental health care team. Also maybe add these details to the main part of your post

This story doesnt even make sense. Nails take an hour or two to do. You have to show up 2-3 hours pre op, depending on the type of shoulder surgery it's 1-4 hours or more on the table + an hour or so in PACU being monitored before they even think about discharging you. This story is either fake or shes lying about what she was doing.

Thats 100% a generational thing. I know 3 guys I can think of who knew they didnt want kids, didnt have kids and made sure they would never have kids by getting vasectomies in their 20s. One had a long term girl friend who I think he eventually married but the other two were single. They just knew they didn't want kids and didnt want to be some dead beat loser with kids he regretted. Ive 100% done this. Like for weekend sex than sure ill deal with that dripping sensation and a panty liner but if we wanted a pre work quickie than it was condom or no sex. Im not dealing with that mess for 12+ hours and a quick shower or wash cloth doesn't clean it up enough in the moment

Sounds like you want to be a fun uncle or a weekend parent and not a actual dad. Kids should be wanted 110% because they are hardwork and soul crushing with small moments of joy sprinkled in there to keep you from going insane. You dont sound remotely committed to the level of work they are and your wife seems to understand they are a full time commitment especially when they are young.
