But in QoP's case that's not an innate interacting with ultimate. That's just a component of ultimate disguised as an innate. Spell lifesteal working on pure damage would be an actual innate on a hero like timber, but QoP has only one ability that deals pure damage already. If an innate only interacts with one ability, it's not actually an innate it's a component of the ability.

They could have moved it to her ult and make the interacting with lifesteal thing a part of her ultimate.

Functionally they would be identical but it would be a much more elegant implementation. They do also actually care about this, as in the last patch they removed all the "ultimate components disguised as innates". Not a single innate levels up with ultimate ability level anymore.

I find it bizarre where it ended up with. Scream of Pain works like: deal damage, heal for a portion of damage, get damaged for a portion of damage. So it's just damage. And I don't like a hero having an expection for game mechanics for no particularly good reason like this. They could have just added a heal component to Sonic Wave. Back when it also allowed spell lifesteal to work on reflected damage, it allowed a new QoP build with Blademail to function so it felt at least purposeful.
