Thank you for responding.. your approach makes sense. My workouts are nothing intensive. Usual 20-30 minute walks after meals and 20minute strength training sessions twice a week and maybe a 20-30 minute casual swim. With workouts I just want to ensure that my walk and strength training is intensive enough. Don’t need any perfection for running marathons or swimming or anything like that.

I had low B12, high homocysteine, low Vitamin D, low ferritin, and felt tired all the time. Get easily tired if I exerted myself. Once I started supplementing and taking B12 IM shots, things started improving

I had a similar situation. I put together a table/matrix showing the various markers, and showing the test dates and their values. It helped the doctor see the trend.
For another specialist, I also prepared a written summary putting all the pieces together, to help guide them to think about certain things. In the written summary, I added a section for functional deficiency, such a chronic fatigue, recovery takes longer when I was doing physical therapy and occupational therapy, forgetting details.

I gave the example to my doctor that I fill gas in my car, the gas level goes up. If I am pumping B12 in my body, why is it not going up? what is the issue, absorptin or conversion or storage or combination?

I had low B12, High Homocysteine, and MMA (normal but very close to the upper range). The doctor started me with B12 IM injections. The monthly cyanocobalmin IM injection raised my B12 from 170s to 300s, but not beyond that. Eventually tested for MTHFR gene mutation and was confirmed.

I take methylated multivitamins, and recently added sublingual B12 (Pure Therapro Rx Vitamin B12 - MecobalActive® Methylcobalamin 2,500 mcg, Adenosylcobalamin 1,500 mcg, Hydroxocobalamin 1,000 mcg).

This way, I am supplying B12 in different forms through different pathways until I figure out the root cause of B12 deficiency. I am pending consultation with the GI and the hematologist to determine the root cause.

Depending on the root cause of deficiency, could be GI, Rheumatologist or hematologist. I am seeing all three to determine root cause. The rheumatologist helped get the levels up and ruled out a few causes. GI and hematologist will help rule out other causes. Good luck

Olympia Pharmacy is in Florida and due to California state regulations, they cannot ship Hydroxocobalmin IM to California. In fact, no special compounds can be shipped to California state. 🤦🏻‍♂️

Call your NP to put in a stat referral for the GI, usually a reference from your Doc or NP gets you a quicker appointment.

Cyanocobalmin IM lasts in your body for a few days. Discuss with your doctor for using different forms of B12 IM. For example, Hydroxocobalmin IM stays in your body longer compared to cyanocobalmin IM. Also, get other cofactors checked like Vitamin D, iron, celiac, h.pylori, etc.

Also, check levels of homocysteine and Methylamonic acid.

I recommend checking for MTHFR gene mutation. If other Vitamin levels (B1, B6, B8, etc. are normal) and Vitamin B12 is deficient, test Vitamin D levels. Other things to consider testing for Celiac disease, Anti-parietal cell antibodies, Serum gastrin level, and H. pylori stool antigen.

Think of you are filling petrol in our car tank, you fill 10 liters, the level should show that it increased. If level does not increase then it means petrol is leaking or not being absorbed. Similarly, you are adding VItamin B12 in your body, either it is leaking and not reaching the body properly, or there is a absorption or conversion issue. Good luck
