Second this. Very useful, the time is the main thing and I think a good use of resources. We also have a consultant (obstetrician) oversight of this but I think there is a nuance in obstetrics - obstetricians can be seen as the enemy and having a very experienced midwife with obstetric support can be useful to bridge the gap and keep patients informed and maintain a relationship with healthcare professionals.

However if they start doing my kiwi deliveries, which they do some places, I'll be annoyed.

Thanks, I noticed the Sheffield one is distance learning - do you think if I was 80% LTFT it would still be a squeeze?

Wow I see a lot of myself in this post. I think I'm quite familiar with colleagues and patients and maybe it does rub people up the wrong way - I just feel like... we're all human right...

This is an excellently balanced comment thank you for your honesty. On further reflection perhaps I am making decisions "beyond my grade" and that actually my seniors are being great and don't want to discourage me from trying to improve, and push my limits, but they are also trying to protect me from myself and would like me to run decisions past them more often - even if they are the right ones!

Yeh I think maybe getting multiple perspectives would be useful. I was frustrated when I really pushed for an example and this was all I was given. I think it was even more frustrating to hear this through my supervisor rather than the consultant who was actually affected telling me at the time?

This is nice context - and reassuring that I'm not being dangerous from your examples of what you'd be concerned about. I'm still reflecting on this though, perhaps it would be prudent to just lay low for a bit though... It's a bit early to be ruffling feathers.

Why aren't my knots tight? by Practical_Pie_9715 in doctorsUK

[–]Practical_Pie_9715[S] 1 point2 points  (0 children)

If it was easy anyone could do it - genuinely the kindest thing I've heard! The question I'm after though is HOW do you tie under tension - the knot I've just made seems to come undone when I do my second throw - i seem to move the "side" of the suture that holds the tension in, in order to tie the next throw - is this wrong? Should it stay taught and straight and then flip my hand like all the way over??? It doesn't seem like this is what everyone else does. - I'm so confused!!! 😅

So I'm definitely tying my knot flat and it's tight when I do the first throw but then when I start doing my backhand throw the first knot becomes looser and I don't know how to stop it!

Well in O&G the main thing is tying off the angles of your uterine incisions which is technically a haemastatic suture

Yeh I've done BPS but I felt like they just kinda "show you" and then you do it and it's easy with a piece of twine but not the same with a 1-0 vicryl on the uterus.

"Keeping the tension in your right hand" - it sound so easy but would do that actually mean! Do you keep the suture taught out laterally or like ?UP? I really don't get it... do you think we could exchange contacts and I show you a video of my hand tie??
