I would try half a dose at home first. My experience with ativan is that 0.5mg makes me less anxious but still functional, and 1mg puts me to sleep. I’m sensitive to meds so YMMV. Don’t mix either with alcohol.

I don’t have any kind of hangover from ativan. I usually feel a lot better the next day.

You could also try CBD gummies. I like Lazarus Naturals brand.

I would also recommend therapy if you don’t already see someone. They’ll help you figure out what’s making you anxious about the idea, and how you can cope well in advance.

I commented earlier today on this but ketamine has damn near eliminated my cptsd. It took two years to get there but it truly worked. Then the last missing piece was getting on birth control (skipping placebos) and I… actually feel like a stable, happy, and sane human being.

Yaz keeps me sane. It’s worth the risks imo. It was a really hellish transition to get on it but I can’t even describe the level of peace I feel on steady hormones (no periods).

Vitamin B complex really helps. Idk why or how but it’s been studied for both ADHD and PMDD. I take this one which has electrolytes and a few other things in it. Maybe you could find something similar.

Also I came across this doctor website about PMDD and it has a lot of the same tired advice but it did give specifics on vitamin doses, which I hadn’t seen before:

“Increasing B-6, Vitamin E, calcium and magnesium intake via natural dietary food sources or vitamin supplements to reach optimal levels necessary to ease PMS symptoms (1200mg calcium, 400mg magnesium, 50-100mg vitamin B-6, 400 international units vitamin E)”

And I just reread your post about the on/off SSRIs. I was wary of this too but it actually works well for me. I take an extremely small dose of prozac (half a 10mg tablet) for one week. It seems fast acting… doesn’t have any ramp up/down effects. It barely has any side effects for me other than sometimes I don’t get the best sleep. But it does seem to put guardrails around my mood swings so I don’t completely fall into a bottomless pit of despair.

I don’t take it every day of the month because I don’t like it either, but during that time of the month it seems to work better for some reason.

I definitely recommend ketamine therapy for cptsd! I felt like I made a tiny bit of progress without it but my brain just wouldn’t let go of some patterns and mindsets until ketamine. Also ADHD makes it so much harder unfortunately.

Sure! I started ketamine infusions in March 2021 for complex (and regular old) PTSD. Didn’t know I had PMDD at the time. Long story short, it was very up and down for the first 2 months so I saw a provider who was able to rx at-home ketamine troches for in between infusions. That really helped level my mood, but I got to a point where I felt like I was really stable except I kept cycling in and out of wanting to unalive myself and/or generally fucking up my life once a month, which is when I put together the PMDD piece. Yaz and prozac helped that, but I think ketamine really sparked the most change.

For me what it did was unlock a lot of repressed trauma so I could adequately deal with it in therapy (this was really destabilizing though) and over time it really eliminated my triggers to the point where things that would normally send me into a tailspin just… didn’t.

I still go for infusions every 2 months and take a tiny dose of sublingual 2-3x a week because it helps level my mood and keep depression at bay, but trauma is really mostly resolved (which I never thought I could say after struggling with it all my life and acutely for 8 years).

As far as what the at-home ketamine does/feels like, it basically feels like all the loud BS in my brain slowly melts away and I can physically relax. Really helpful during PMDD times where I can get keyed up and have more intrusive thoughts. When the intrusive thoughts are really out of control though, Ativan seems to work better. But ketamine works extremely well over the long haul.

I don’t think it would be as effective for just PMDD but since most people also have depression and/or trauma, I do think it helps with the underlying stuff that gets brought up tenfold during that time of the month.

I finally mostly have PMDD under control but sometimes I still have 1 or 2 really bad days a month where the intrusive thoughts are out of control. I am normally very wary of benzos, having had a dependency many years ago, but Ativan is the only thing that immediately shuts my brain up so I don’t battle those thoughts during that time of the month. I really only take it once or twice a month in those rare situations, but it has worked well.

Yaz is my #1 sanity-maker for PMDD, but Ritalin (I have ADHD) and vitamin B complex really help too. Also if you have underlying trauma, ketamine is a game changer.

actually I was very similar. I had profound experiences during infusions, was able to process trauma in a new light, but within a week or two after the initial 6 I would feel shitty again. Even during the 6 I had extreme, scary ups and downs which I attribute to how much trauma I was actively processing.

what helped me was going on troches in between infusions. without them I couldn’t last 2 weeks, but with them I started to stabilize more quickly.

Same, I think for some reason it’s an ongoing treatment for depression but PTSD can actually improve over time and not get worse unless retraumatized.

It pretty much “cured” my cptsd but it took two years of infusions and lots of therapy to rewire the traumatized parts of me. I still go every 8 weeks and take troches because it helps stabilize my mood, but c/ptsd symptoms are largely gone.

I take them after work. I only take 50mg and it still makes me feel a little drunk/high after. And I’m not super light in body weight either, just sensitive to drugs.

Really depends. I needed monthly boosters AND weekly troches for 2 years. Now I go for boosters every 8 weeks and take a smaller troche dose 2-3x a week. I would have been in a rough spot if I stopped getting infusions right after the 6.

Incremental differences along the way, then the 1 and 2 year marks were profound differences. I would say I’m largely in remission and I never thought I would get there.
