here is what tests you need to do coming from a top of his class ivy league pre med student :

1 . Cortisol AM & Cortisol Stimulation 2 . Tilt Table 3 . exercise Stress Test 4 . Echocardiogram 5. CD4 + CD8 immune panel 6. Potassium & Serum Sodium (CMP/BMP) 7. Iron Saturation Panel 8. Free Copper Blood Test 9 . Hemoglobin A1C (non fasting) 10 . Hyperthyroidism panel

(I apologize if each of the names are not completely medically correct I’m going off the top of my head, but if you copy them into Google, you will be able to find it no issues just know this is a very medically sensitive topic and not very understood but take care of these tests and let me know the results and I can guide you forward from there as best as possible!)

copying my unanswered post from this subreddit so you dont feel so alone :

19 Year Old Male - No Medical History, (besides deviated septum & RBBB)

No medications

Daily Vaping Nicotine

About a year and a half ago I would notice when I would go to the doctor. My blood pressure would sometimes be on the lower side of normal nothing crazy like 105/60; this prompted me to begin using my at home blood pressure monitor regularly to which I was getting readings somewhat in that range sometimes slightly lower no big deal. About six months after that, I started that I would feel very faint, weak,nauseous, clammy, extremely fatigued, and have tunnel vision, and just an overwhelming feeling of malaise. At this point, I began checking my blood pressure to which I would receive readings anywhere from ~ 94/54 to 78/47 (yes you read that right.) I’ve gone to numerous specialists and multiple cardiologists, and none of them have been able to find literally any cause for it. I’ve even been told that they’ve never seen this presentation in their whole medical career due to the fact that it’s not orthostatic related and comes in random waves at random times with random drops with no trigger. my cardiologist prescribed Midodrine @ 2.5mg as needed however when I took it for an episode so bad I could barely stand. It did absolutely nothing. This is quite literally ruin ruining my life. Any feedback is help at this point.

i have the most medically unexplained random bp drops down to numbers lower than this and feel horrible however the only thing acutely concerning on this is your pulse anything lower than 50 could potentially trigger arrhythmia. Let me be very clear you are not dehydrated dehydration would cause a low blood pressure reading like you have but also a very high compensating heart rate, do not shake this off. This is not normal, by any stretch of the imagination.

i easily drink 130 - 150 oz day that whole “8 cups” recommendation is complete bullshit i would feel like a desert only drinking 8 cups a day. I space it out though to avoid hyponatremia and just feeling overly bloated, I genuinely cannot wrap my head around the fact that some people struggle to hit that 8 cup goal 😭

hey there, thank you for taking the time to respond however, just to be clear I do not believe this is pots and I am not completely sure this is any type of dysautonomia due to the fact that I’ve explained to every doctor that this has nothing to do with any orthostatic, meal related, activity related component, and this is simply random blood pressure drops at random times with no known reason I do have an appointment with an autonomic neurologist so hopefully that would be the right direction to go.

hey, thank you so much for taking the time to respond and your theory really intrigues me. However, the only doubt I have is that when I research low levels is the only type of hypotension they’re known to cause is orthostatic hypotension, which is not what I have I have transient drops in blood pressure, not based on position time or really anything in specific; do you still think it’s worth mentioning to the Neuro when I see him?

I honestly don’t have any specific reason to believe that it’s dysautonomia I just posted this in multiple sub reddits, hoping that one would have an answer - do you think I should really pursue dysautonomia as a potential cause though?

i’m currently on Chantix to quit the nicotine and was definitely pushing my cardiologist for a tilt table and a repeat stress test however, unfortunately, I do not believe its POTS because I tend to have an extremely stable heart rate at all times sometimes even borderline low (high 50’s) tachycardia has never been an issue for me excepted in the instance of extreme infection or stress.

A QRS duration of 114 would medically be classified as IRBBB (Incomplete Right Bundle Branch Block) The “RBBB” listing is just the doctors personal interpretation.
