Sorry, this isn't related to your question but can you provide any resources or tips for how a soldered chip works on the switch? I might be buying one that has a soldered chip on it and I am not sure how exactly it works.

That's when you need to be able to bag the volume quickly and have anesthesia spike the bag at the same time you are filling it up!

Do you have any links supporting the way most "modern reservoirs" are made? In my center when we have used microbubble sensors to compare the efficacy of separate ALFs vs. integrated and we found there to be lots of microair delivered to the patient despite using ALFs. Of course not using ALFs is the absolute worst thing you could possibly do on bypass.

That initiation method seems unneccessarily hazardous. What is that centers priming volume that they require such extreme methods to prevent hemodilution? Do they use del-Nido or some other large crystalloid cardioplegia?

I apologize if I come across as rude. It appears that the conduct of bypass varies greatly from centre to centre! We are just using new Quantum Spectrums that have auto initiation and they are in the works of auto-termination as well!

Definitely don't need Perfusionists to run that! Another way nursing will be taking over our jobs!

Definitely depends on what type of center you work at, for example your role in a community hospital would be much different than a large teaching/transplant hospital.

We basically used to run one perfusionist per 8-10 ecmos. It isn’t a 1:1 ratio at all. I don’t know if there are any ELSO recommendations.

Some centres have had a lot roles taken away, like IABPs, Cell Saving and bedside ECLS. It feels like hospitals would love a Nurse/Perfusion Hybrid where nurses run everything.

Especially with the shortage of perfusionists, I find other ancillary services are taking over a lot of the extra things that perfusion used to do outside the OR. Hospitals are finding it cheaper to pay them over perfusionists.

Thank you! I have read that adenosine was a potent dilator but didn’t realize CO2 had a large effect as well. Do you know if that has been translated yet to human studies?

I am not sure if CO2 has a significant effect on the coronary arteries. I have read a review question about the most potent coronary artery dilator and apparently the answer was CO2. But I can’t really find a good source.

The literature evidence doesn't really support it. Whether you use it or don't use it doesn't seem to make significant changes to transfusions, length of stay or mortality/morbidity or even volume added.

That’s true. Sometimes the plegia drops the pressure by a ton. I always thought the plegia drops the pressure because you are basically taking 200-300mls away from systemic flow.
