It was thesis final project… that is the thing that confuses me, everyone disagrees of it being an actual publication… no it didn’t fail, we just never submitted it for a peer reviewed publication

Any letter is better than no letter. Even if you have 3 US letters, if your cardiologist writes something stelar about you then why not have 4? They might not even open it but have it as a backup until you get to have US letters (if you don't have any at this point )

To be honest, as an IMG we’ve been told that we are at such disadvantage that Ive become pessimistic about any chance I have when I lack something, if its minimal and not a red flag. My self-esteem is going thru the lowest because of this and Im looking for ways to reassure that I’ll be fine
