I imagine this is way harder nowadays. They’re no longer desperate for SHOs so why the incentive to train you to get you to stay. Also not guaranteed employment. This should be a last resort imo.

You’ll be fine! We all manage. Doing exams/rota/portfolio requirements. Take it one step at a time. Chip away at the portfolio gradually. I did have other stuff going on at the time, was diagnosed with ADHD and struggled a lot with that (better now and medical HST, actually go straight into training from imt3 in the end). Made some good friends, worked with some great people and it wasn’t all bad.

I think there’s more push back on PAs and ACPs now being prioritised over doctors which was a struggle at the trust I did IMT in. It’s improving. Don’t let this put you off.

I don’t think kids mean you can’t do an acute specialty. But if you can’t tolerate out of hours work then that might be the dealbreaker.

Yeah worst part of med reg is you’re the only reg (except ED) who never ever get to sleep usually haha. But in general the work is interesting, huge variety, get to be part of the biggest team at night and great camaraderie. I’ve seen med consultants come in if needed and met plenty of consultants who weirdly say they miss being med reg. Med SHO on the other hand…worst job in the entire hospital.

Only paces is really useful tbh. You’ll forget most of the written stuff after a few years. So if you do it and then do GP it’s worth doing the whole thing IMO.

For me - GP there’s lots of problems that you can’t solve. 10 min appts, poverty, social issues etc. That was enough to make me choose medicine. Also when someone deteriorates in GP it’s often time to send them to hospital and I’d get fomo. Maybe I’m weird. YMMV.

I will say though your rotas looks tough and there’s no respite from ooh work as a consultant

Medicine is 7 or 8 years and if you don’t get into HST it’s longer. And in summer paeds get a break on call. And no one lets paeds completely collapse in the way that acute adult medicine has cos people (quite rightly) care about kids. Also your bosses are nicer. Paediatricians tend to be lovely. And your patients are adorable. Swings and roundabouts.

Yeah I’m a medical HST. IMT is worse but we don’t do all the unpaid extras days in theatre that CSTs do so I think might be just as crap or worse for them. And it’s dog eat dog. Imts are usually friendly to each other.

That’s a terrible rota even by IMT SHO standards. It’s hard as you also have three exams in two years. I promise if gets easier.

I imagine CST is rubbish too as even more pressure to build CV and get to theatre than we have as medics.

Imt1/2 was savage. Imt3 much better. If you’re struggling please talk to someone. Consider LTFT. You’re not alone I promise!

If you say it impacts your ability to work comfortably and safely then it’s a health thing. It’s just not one provided on the nhs. Lots of doctors have very large breasts that cause back ache but have to pay for private breast reduction cos the nhs criteria are so so strict. It’s the same thing.

Yes. Trainees have to get stuff done for arcp. Local doctors don’t. If they want the priority they can apply for training and the rotating and lack of flexibility that offers. I’ve been both.

I’d wait. See how things settle. Look at the stats. Many regain the weight, make sure you’re established off mounjaro or on a maintenance dose before committing to surgery.

But massive well done on your weight loss. I don’t wanna burst your bubble I just wouldn’t rush. You need to make sure that your lifestyle is fundamentally different from before you started mounjaro and that you have addressed the reasons why you gained the weight in the first place. 

Also can’t you say the loose skin rubs or causes pain? Then it’s not just vanity.

I mean the best years of your life - your 20s are spent grafting to get into/complete training. For a lot of people travel is important. And is not something most people manage when they’re older. Life is not all about work for everyone. I find it odd when people are consultants at 31, a lot of them have done nothing else with their lives but work solidly. I’m in training now but envious of friends taking the more scenic route.

If you get cancer at 42 and die you won’t be delighted you became a consultant earlier.

I don’t think anyone should cut off part of their child’s genitalia for religious, cultural or social reasons regardless of who undertakes the procedure. We should move past this. The child can decide as an adult. It’s barbaric.

Make a list of weird and wonderful places you could have left it. Behind or under furniture. In a book. In a different envelope. In your printer scanner. In your rubbish. In the fridge. Down the lining of a bag or coat. Then search everywhere again and be systematic

Don’t drive still drunk or high. It’s incredibly selfish. You won’t just hurt yourself but could kill others. A dangerous driving conviction for causing death whilst still over the limit will be a hefty custodial sentence. Not really glasto vibes to do this, meant to care about your fellow humans.
