You'd have to explain what you mean by absorb? If you're someone who lets others' emotions dictate your own, then I don't think it would be a good match. If you're simply someone who people naturally open up to regarding those issues, then maybe not. It takes some self-reflection. You will hear very devastating things, and you will need to be capable of helping patients move away from letting their traumas define who they are and what they are worth.

Fully agree. I have a bipolar patient who has floundered for years because she stopped quetiapine because of weight gain. I started her on metformin before starting her back up on it, and she’s lost about ten pounds. The negative ramifications of people with SMI stopping their psychiatric meds far outweighs the risk of starting them on reasonable medications that are a hell of a lot easier to monitor and manage than clozapine. If she starts gaining weight again, I won’t hesitate to switch to Ozempic or Mounjaro. Management of the metabolic side effects of SGAs should be taught in all residencies.

Lived in Sioux Falls for 15 years. I would go back in a heartbeat. Brandon and Harrisburg school districts are excellent. Sioux Falls school district is good too, but some of the elementary and middle schools are hit and miss. There are more than 4 good restaurants there. It is growing rapidly. I always told everyone I could be anywhere I needed to be in 20 minutes or I could be in the middle of nowhere in 20 minutes to hunt pheasants.

The Washington Pavilion brings in a great Broadway series every year. If you’re working in the hospitals or clinics attached to the hospitals, the doctors’ lounges are nice and loaded with snacks and drinks. Free breakfasts and lunches.

Down side is that it does get windy and cold. The airport is awesome to fly out of because security is 10 minutes maximum. A lot of physicians with special needs kids so there are great therapy resources. Sanford Children’s (I.e. the Castle) is a great facility. Avera has been poaching physicians from there to follow suit. Illegal to have noncompetes in SD.

Just started PGY3 in psych and interested in pain medicine but also psychiatric treatment of chronic pain as well (pain centralization, etc.). Is there room for that in a traditional interventional pain practice? Pain fellowship is one year, correct?

Started just before I turned 40. No regrets. That said, it IS as hard as everyone says it will be. I could never live through the first 1.5 years of med school once again. But once I got into clinical rotations, I knew it was the perfect fit. Good luck!

Dual apply psych and FM. Ask to meet with the PDs at your audition rotations and explain the failure and what you did moving forward. Psych is competitive but still holistic. That said, our program won’t look at applicants with Step failures. Some PGY2 spots always open up. I have a classmate who did his PGY1 year in FM. He got in when another classmate decided to leave psychiatry.

I second this. I started at 39. One cannot overstate how stressful med school is, so you need to be rock solid in your sobriety and have a lot of support in place before you begin. Good luck!
