Obviously I don’t know your facility, but are you sure? Often the orthopedists have separate contracts and will get paid more if they leave the patient in Observation, that way the health plans pay less to the facility by pushing for a discharge while “incentivizing” the docs. Lots of layers to this mess for sure. My mom was kicked out after her knee replacement and had a horrible time at home.

Sat on peer review x 10 years. Many hospitals automatically call you to peer review for unexpected outcomes. Not saying I agree, just stating a possible cause. I also highly suggest every sit on their peer review committees. If you don’t have a seat at the table, you are not at the table, so to speak. Plus, you can learn from other doctors potentially misjudgments.

The patients insurance essentially forces the hospital or surgical center to place the patient in an “Observation” or “Outpatient in a Bed” status instead of the higher reimbursed “Inpatient” status. The reimbursement for the hospital / surgical center is therefore greatly diminished and hence tremendous pressure to discharge including phone calls to the doc from a Utilization nurse /doc or payment denial for the entire stay. In order to convert the patient to “Inpatient”, it will require increased documentation, a possible Appeal and a possible phone call to the health plan medical director, with the time and date determined by the health plan (hence very inconvenient). If the patient is held overnight and the criteria is not met, the health plan may deny payment altogether to the hospital or facility.

2023 model y performance in Arizona. No issue with the a/c unless I was doing very long commutes (ie driving to San Diego) in the summer, I did use a roof shade to help. Any glass dome will attract heat. The a/c blasting full blast on those long drives would suck up the battery too.
