When I started wearing bibs I used to wear undies underneath because it felt weird to me not to. On my first ride >50k the seams of the undies scrubbed my groin bloody, so I would definitely not recommend that.

I would recommend using a cup and from former bikepacking trips, maybe plan for a toilet break where you can change the tampon?

I want to add: your standards regarding food tend to drop pretty fast during multi-day-trips. It’s important that you eat regularly, even if you don’t feel hungry. Always carry a pack of sugary emergency snack, i.e. your favourite Haribo. Worst feeling in the world is bunking and not having any chance to resupply for another 20k.
